DIEP皮瓣在乳腺癌术后一期乳房重建中的应用体会

【摘要】 目的:探讨腹壁下动脉穿支皮瓣(DIEP)+微血管吻合术在乳腺肿瘤术后一期乳房再造中的应用价值。方法:选择2007-4-25-2008-2-1收治11例乳腺肿瘤患者,其中保留乳房手术3例,保留或不保留乳头乳晕皮下全乳切除8例。所有患者术后即刻行腹壁下动脉穿支皮瓣+微血管吻合术,对缺损乳腺一期整形或再造。结果:11例乳腺肿瘤患者中,浸润性导管癌4例,浸润性小叶癌1例,导管内癌伴微小浸润3例,导管原位癌2例,恶性分叶状肿瘤1例。11例患者皮瓣全部成活,无脂肪液化、腹壁切口疝等并发症发生,美容效果满意。结论:腹壁下动脉穿支皮瓣+微血管吻合术是乳腺肿瘤术后一期再造的理想术式。

【Abstract】 OBJECTIVE:To evaluate the immediate breast reconstraction with deep inferior epigastric perforator flap after operation of breast tumor. METHODS: From April 2007 to February 2008, 11 cases of breast tumor were admitted. The operative methods included conservative surgery in 3 cases and skin-sparing mastectomy in 8 cases. The breast defects of all the patients were reconstructed immediately by deep inferior epigastric perforator flap (DIEP).RESULTS: Of them, there were 4 cases of infiltrative ductal cancer, 1 case of infiltrative lobular cancer, 3 cases of ductal cancer in situ accompanied with microinvasion, 2 cases of ductal cancer in situ, 1 case of malignant lobulated tumor. Eleven flaps survived completely postoperatively.No complications included fat liquefaction,flap necrosis and incisional hernia occurred.Reconstructed breast achieved good results in shape and symmetry. CONCLUSION: Deep inferior epigastric perforator after operation of breast tumor is an ideal method of the breast reconstruction.
