结缔组织病患者静息能量消耗与呼吸生理指标分析

【摘要】 目的探究结缔组织病患者肺功能及动脉血气等呼吸生理指标对静息能量消耗(REE)的影响效应。方法选择19例结缔组织病患者和21例正常对照者,对其分别进行静息能量代谢和静息肺功能测定,并进行血气分析。结果(1)结缔组织病组患者REE、REE/预计值、REE/体表面积、REE/体质量与正常对照组比较,差异有统计学意义(P<0.05)。(2)两组受试者用力肺活量、第1秒末用力呼气容积、一氧化碳弥散量间差异有统计学意义(P<0.05)。(3)两组受试者心率、动脉血氧分压、动脉血二氧化碳分压、动脉血氧饱和度、呼吸困难指数(BorgScale)间差异有统计学意义(P<0.05)。(4)结缔组织病组患者REE与心率、呼吸频率、口腔阻断压/最大吸气压及BorgScale呈正相关(P<0.05),与动脉血氧分压、动脉血氧饱和度、用力肺活量及一氧化碳弥散量呈负相关(P<0.05)。结论结缔组织病患者REE显著高于正常对照者;除与PaO2和SaO2有关外,REE还与肺功能如FVC、DL-CO等有关。

【Abstract】 Objective To investigate the effect of some respiratory physiological factors such as pulmonary function and blood-gas analysis on rest energy expenditure(REE)in patients with connective tissue disease(CTD).Methods Totally 19 patients with CTD and 21 normal subjects were enrolled.REE,rest pulmonary function and arterial blood gas analysis were respectively conducted for all the subjects.Results The differences in REE,REE/expected value,REE/body surface area REE/body mass between CTD patients and normal subjects showed statistically significant(P<0.05),and so did in forced vital capacity(FVC),forced expiratory volume in first second(FEV1)and diffusion capacity for carbon monoxide of the lung(DLco)(P<0.05).There were statistically significant differences in heart rate,arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(SaCO2),arterial blood oxygen saturation(SaO2),and Borg Scale between the two groups(P<0.05).The REE in CTD patients was positively correlated with hear rate,respiratory quotient,mouth occlusion pressure(P 0.1)/maximal inspiratory pressure(PIMAX),and Borg Scale(P<0.01);however it was negatively correlated with PaO2,SaCO2,FVC and DLCO.(P<0.05).Conclusion Rest energy expenditure were obviously higher in CTD patients than in normal subjects.Besides PaO2 and SaO2,it is also related with FVC and DLCO.
