【摘要】 目的探讨硬膜外麻醉中丙泊酚安全有效的镇静剂量范围。方法择期硬膜外麻醉下行下腹部及下肢手术的患者100例,随机分为5组,各20例。Ⅰ组丙泊酚0.2mg.kg-1.h-1微泵输注;Ⅱ组丙泊酚0.6mg·kg-1.h-1微泵输注;Ⅲ组丙泊酚1.0mg·kg-1.h-1微泵输注;Ⅳ组丙泊酚1.4mg·kg-1.h-1微泵输注;Ⅴ组为对照组,不给予丙泊酚,给予生理盐水1.0mg·kg-1.h-1微泵输注。结果各组患者用药前后及组间MAP、HR变化差异均无统计学意义(P<0.05)。Ⅳ组6例患者SpO2降至90%以下,经面罩吸氧后逐渐好转;Ⅰ、Ⅱ、Ⅲ、Ⅴ组患者未见SpO2下降与Ⅳ组比较差异显著有统计学意义(P<0.01)。Ⅰ、Ⅴ组患者镇静催眠为5级,Ⅱ组患者镇静催眠为4级,Ⅲ组患者镇静催眠为3级,Ⅳ组患者镇静催眠为1级。结论采用0.6～1.0mg·kg-1.h-1微泵输注的丙泊酚有良好的镇静催眠作用,可消除椎管内麻醉患者的焦虑与恐惧或使其在催眠中安度手术,术后遗忘效果满意,对患者的心理健康和生理健康有良好的保护作用。更多还原

【Abstract】 Objective To investugate the dosage range of propofol during epidural anesthesia.Methods 100 patients were randomly divided into five groups,20 cases per group.The patients were treated with propofol 0.2mg·kg-1·h-1 in Ⅰ group,0.6mg·kg-1·h-1 in Ⅱ group,1.0mg·kg-1·h-1 in Ⅲ group and 1.4mg·kg-1·h-1 in Ⅳ group.While in Ⅴ group the patients were given normal saline 1.0mg·kg-1·h-1.Results MAP and HR in five groups was not different after administration of drug(P<0.05).The index of SpO2 decreased to less than 90% in Ⅳ group,and did not change in other groups(P<0.01).The degree of sedation was 5 in Ⅰ、Ⅴ groups,4 in Ⅱ group,3 in Ⅲ group and 1 in Ⅳ group.ConclusionThe effect of propofol at the dosage of 0.6～1.0mg·kg-1·h-1 is good and safe during epidural anesthesia.更多还原
