危重病人肠内营养添加谷氨酰胺的应用效果观察

【摘要】 目的通过给危重病人肠内营养添加谷氨酰胺,观察对危重病人营养支持的效果。方法选择45例危重病人,分成观察组(22例)和对照组(23例)。两组均应用等热量、等氮量的营养支持,观察组在此基础上行肠内营养支持添加谷氨酰胺21d、每天15g,观测两组的体重变化、白蛋白、前白蛋白、转铁蛋白、免疫球蛋白、CD4/CD8、感染发生率等指标。结果观察组的转铁蛋白、前白蛋白、免疫球蛋白、CD4/CD8、与对照组比较有较低的感染发病率。结论危重病患者通过肠内补充外源性谷氨酰胺,可以在一定程度上改善营养支持效果,减少肠道并发症的发生和降低院内感染率。

【Abstract】 Objective To evaluate the effects of glutamine in critically ill patients.MethodsForty-five critical patients were randomly divided into two groups: In glutamine treatment group,glutamine was added enterally(glutamine 15g/d for 21 days).Group B was the control group.The body weight,albumin,transferrin,prealbumin,immune globulin,CD4/CD8 and infection incidence were measured before and after treatment.ResultsThe body weight in the treatment group after treatment was lower than that before treatment(P<0.05),in the control group body weigh was very significantly lower than that before treatment(P<0.01).The transferrin,prealbumin,IgA,CD4/CD8 in control group was lower than those before(P<0.05).No significant changes in two groups of the albumin,IgG and IgM.ConclusionGlutamine administrated by enternal may achieved ideal effect improve critical patient’s nutrition status;increase glutamind can improve patients’ intestinal mucous membrane and decrease the ratio of intestinal dysfunction.
