免眼实验性小梁切除术后应用曲尼斯特的组织病理学和免疫组化的研究

【摘要】 目的探讨曲尼斯特在青光眼滤过术后抑制纤维瘢痕形成的价值。方法27只兔子分为对照组、MMC组及曲尼斯特组,双眼均行标准小梁切除术。MMC组术中应用丝裂霉素C(MMC),曲尼斯特组术后应用曲尼斯特眼水。术后7d、14d、28d各组分别处死兔子3只,眼球组织固定,用HE、Masson三色染色和PCNA和TGF-1免疫组化染色。结果HE染色,对照组术后7d滤过道成纤维组织在滤过道大量增生。Masson三色法染色,对照组的新生胶原纤维排列致密。术后7d、14d对照组PCNA阳性细胞明显多于MMC组和曲尼斯特组(<0.05)。对照组TGF-1染色阳性面积和平均光密度值明显大于曲尼斯特组和MMC组,差异均有显著性(<0.05);曲尼斯特组和MMC组比较差异无显著性(>0.05)。结论曲尼斯特能抑制青光眼滤过术后成纤维细胞的增殖,其作用机制可能包括减少TGF-分泌从而抑制成纤维细胞增值。

【Abstract】 Objective To explore theanti-scar effect of tranilast onthe filtering surgery in rabbits and its mechanism. Methods Twenty-seven rabbits (54 eyes) were divided into three groups: control group, MMC-treated group and tranilast-treated group. Tra-beculectomy were performed on each eye. In MMC- treated group, 0.4 mg/ml MMC were used and 0.5% tranilast eyedrops were used after operation in tranilast treated- group. Three rabbits of each group were randomly killed on the 7th, 14th and 28th day after opertion respectively. Conjunctiva and sclera of filtering area of the rabbits were fixed by 10% formalin, then were embedded. H-E staining and Masson-trichrome staining were aplied in the pathologic histology examination. Proliferation of fibroblast were observed by im-munohistochemistry staining of proliferative cell nuclear antigen(PCNA) and TGF- 1. Results Compared with MMC-treated gro-up and the tranilast-treated group,there were more fibrous and granulation tissue in filtering channel in control group 7 days after op-eration. There were significant differences of the numbers of PCNA-staining positive cells among the three groups( <0.05). There were more positive cells in control group than those in other two groups( <0.05). There was no difference between MMC- treated gro-up and tranilast- treated group( >0.05). The positive area of TGF- 1 and average optical density in control group were smaller than those in other two groups ( <0.05) and there was no significant difference between MMC-treated group and tranilast-treated group ( >0.05). Conclusion Tranilast can inhibit the proliferation of fibroblast in glaucoma filtering surgery. The mechanism of the ef-fect includes inhibiting secretion of TGF- and antiproliferation of fibroblast.
