整形外科复杂创面的负压吸引治疗

【摘要】 目的:探讨局部负压吸引技术处理整形外科复杂创面的方法和效果。方法:总结2007年~2009年笔者因复杂创面采用负压治疗的患者53例,男性33例,女性20例,年龄26~71岁,创面面积6cm2~80cm2。结果:2例负压治疗后,新生上皮组织覆盖创面,治疗开始至愈合时间均为21天;51例行负压治疗后创面情况明显改善,二期行植皮或组织瓣转移等常规方法关闭创面(其中15例负压治疗后行全麻下清创),术后创面均一期愈合,治疗时间最短者14天,最长者30天,平均治疗时间19天。结论:负压治疗是创面治疗的一次新进展,操作简便,易于掌握,能够彻底去除创面分泌物和坏死组织,优化创面条件,为整形修复提供良好组织基础,加速创面愈合。

【Abstract】 Objective To introduce the method and effect of negative pressure treatment on complicated wound in the field of plastic surgery.Methods A total of 53 cases(33 males,and 20 females) with complicated wound who received negative pressure treatment were studied.The age of the patients ranged from 26 to 71 years,and the area of wound ranged from 6cm2 to 80cm2.Results In 2 cases,the regenerative epithelial tissue covered the wound after negative pressure treatment in 21 days.Other 51 patients had the wound improved obviously after negative pressure treatment,and then had regular methods such as skin grafting or tissue flap transferring to reconstruct the wound(15 of which had debridement under general anesthesia after negative pressure treatment).All of the 51 patients healed by first intention.And the treatment took 14 to 30 days with a mean time of 19 days.Conclusion Negative pressure treatment is a wound therapy that has been developed recently.It is easy to operate and master.It can remove the secretion and necrotic tissue of wound,optimize the condition of wound,prepare satisfied tissue bed for surgery and promote the healing of wound.
