冠心病心绞痛患者超声心动图变化与其脉象要素的关联研究
Study Ultrasound Cardiography Characters of Pulse Elements in 195 Patients of Coronary Heart Disease
【摘要】 目的:探讨195例冠心病患者多谱勒超声心动图变化特征与脉象要素的相关性,为发展无创的脉诊检测技术提供理论支持。方法:运用M egas多谱勒彩色超声仪采集冠心病患者心脏彩色多谱勒超声心动图指标。结果:冠心病患者脉象要素-脉位、脉力、脉紧张度、脉流利度4方面与超声指标存在不同程度的相关:冠心病脉有力者表现为主动脉流速、肺动脉流速升高,二尖瓣E峰降低;脉紧张者表现为主动脉流速、肺动脉流速增大,而左室舒张末期内径、左室收缩末期内径、二尖瓣E峰、二尖瓣A峰减小;脉滑者表现为主动脉流速、肺动脉流速减低,二尖瓣A峰、射血分数增高;冠心病脉位浮者表现为心脏室间隔厚度、左室后壁厚度减小,主动脉流速、肺动脉流速、二尖瓣E峰有降低趋势。结论:结合多学科研究的手段,可寻找到冠心病患者脉象构成要素的量化指标。
【Abstract】 Objective:To explore ultrasound cardiography characters of pulse elements in CHD,they provide theoretic proof for developing non-invasive technique of pulse diagnosis.Methods:Using Megas Doppler multicolor ultrasound instrument,Ultrasound Cardiography of Coronary Heart Disease Patients were collected.Results:There existed correlation to some extent between four elements of pulse(location of pulse,strength of pulse,tensity of pulse,and volubility of pulse)and heart index with Doppler ultrasonic imaging in CHD.pulse with powerful feeling mainly manifested in Doppler ultrasonic imaging of heart with heightened flow rate of aorta and pulmonary,as well as degrade of E-peak of auriculoventricular vavle(AV).Pulse with tension feeling in CHD mainly showed in Doppler ultrasonic imaging of heart with increase of flow rate of aorta and pulmonary and accompanied with diminish of left ventricular end-diastonic dimension(LVEDD),left ventricular end-systolic dimension(LVESD),E-peak of AV and A-peak of AV.Pulse with smoothness feeling in CHD mostly have presentation in Doppler ultrasonic imaging of heart with slow down of flow rate aorta and pulmonary,at the same time,increase of A-peak of AV and ejection fraction(EF).Pulse with floating feeling in CHD have character in Doppler ultrasonic imaging of heart with thinning of interventricular septal thickness(IVST)and left ventricular posterior wall thickness(LVPWT)and accompanied with stepping down flow rate of aorta and pulmonary and E-peak of AV.Conclusion:Combining with multidisciplinary study,quantizing index of elements of pulse in CHD can be founded.
