【摘要】 目的研究肝源性溃疡(HU)患者胃黏膜表皮生长因子(EGF)、前列腺素E2(PEG2)的表达及其意义。方法选取HU患者30例及肝硬化非HU、慢性胃炎、非溃疡性消化不良(NUD)患者各40例,行上消化道内镜检查,取胃窦及胃体黏膜组织各1块,用ELISA及RIA法分别检测EGF及PEG2含量。结果(1)HU患者胃黏膜EGF、PGE2含量明显低于肝硬化非HU患者。(2)HU患者胃黏膜EGF、PGE2含量明显低于慢性胃炎及NUD组,而后两者无明显差异。(3)HU患者胃黏膜EGF、PGE2含量与肝功能分级无关。结论(1)HU患者胃黏膜防御屏障减弱。(2)HU患者胃黏膜EGF、PGE2的降低参与HU的形成。更多还原

【Abstract】 Objective To investigate the expression of gastirc mucosal EGF and PGE2 in patients with hepatogenic ulcer(H) and its clinical significance.Methods 30 patients with HU,40 patietns without HU,40 patients with chronic gastitis(CG) and 40 patients with non-ulcer dyspepsia(NUD) were randomly involved in this study.The diagnosis of cirrhosis and HU in every patient was confirmed by a combination of clinical biochemical,imaging and endoscopic methods.The severity of cirrhosis was assessed by Pugh’s modification of Child’s criteria.All patients had undergone upper gastriointestinal endoscopy,performed with an Olympus EVIS 240.During endoscopic examination,one mucosal biopsy specimens form the gastric antrum and one from the gasric body were taken from every patient.The levels of EGF and PGE2 were measured with enzyme-linked immunosorbent assay(ELISA) and radioimmunoassay(RIA).Results The result showed that(1)The level of EGF and PGE2 in gastric mucosa in the patients with HU was much lower than that in the patients without HU.(2)The level of EGF and PGE2 in gastric mucosa in the patients with HU was much lower than that in the patients with CG or the patients with NUD.In addition,there was no significant relationship between the level of gastric mucosa EGF and PGE2 in HU and Child’s classification.Conclusion(1)The gastric mucosal barrier in HU is more fragile.(2)The abnormal gatric mucosal EGF and PGE2 in HU plays an important role in the mechanism of HU.更多还原
