【摘要】 目的:通过检测急性肺栓塞(APE)患者血浆中肌钙蛋白I(cTnI)及脑钠肽(BNP)的浓度,探讨联合检测cTnI和BNP对APE患者进行危险分层及预后判断的临床意义。方法:根据血浆cTnI和BNP水平将65例APE患者分为3组,Group1组(15例):cTnI<0.1μg/L且BNP<100ng/L;Group2组(30例):cTnI≥0.1μg/L或BNP≥100ng/L;Group3组(20例):cTnI≥0.1μg/L且BNP≥100ng/L,分析cTnI和(或)BNP升高对APE患者危险分层与临床预后的关系。结果:Group1组、2组、3组发生临床不良事件分别为0(0%)、7例(23.3%)、11例(55.0%),差异有统计学意义(P<0.01)。65例APE患者中发生右心功能不全组与无右心功能不全组比较PaO2、P(A-a)O2、cTnI、BNP水平差异均有统计学意义(P<0.01)。相关分析显示BNP与cTnI、P(A-a)O2均呈正相关(r分别为0.46,0.49,均P<0.05)。结论:联合检测生物学指标cTnI和BNP在APE患者早期危险分层、指导临床决策及预后判断中具有重要价值。更多还原

【Abstract】 Objective:To investigate the clinical value of cardiac troponin I (cTnI) and brain natriuretic peptide (BNP) for risk stratification and prognosis evaluation in patients with acute pulmonary embolism(APE).Method:A total of 65 patients with confirmed APE were divided into three groups,according to degree of serum cTnI and BNP:Group1 (n=15):cTnI<0.1 μg/L and BNP <100 ng/L;Group 2 (n=30):cTnI≥0.1 μg/L or BNP≥100 ng/L;Group3 (n=20): cTnI≥0.1 μg/L and BNP≥100 ng/L. The relations between cTnI and BNP,and risk stratification and prognosis evaluation were evaluated in the three groups. Result:Patients with prognosis mala in group1,group2 and group3 were 0 cases (0%),7 cases (23.3%),11 cases (55.0%)respectively,with a significant difference (P<0.01). There were significant differences in arterial blood PaO2,P(A-a)O2,cTnI and BNP between the group of proceeding right ventricular dysfunction and the group of non-right ventricular dysfunction(P<0.01). Highly positive correlation were observed between BNP and cTnI,P(A-a)O2,PaO2(r=0.46,0.49,0.56,0.39,P<0.05).Conclusion:Combined detetection of cTnI with BNP has important value for early risk stratification and prognosis evaluation in patients with acute pulmonary embolism.更多还原
