掺铒铬钇钪镓石榴石激光去腐备洞技术的临床评价

【摘要】 目的:应用掺铒铬钇钪镓石榴石(Er,Cr:YSGG)激光去腐备洞技术在减轻疼痛、充填后远期效果及工作效率等方面的临床评价。方法:选择口内同时存在2个龋坏程度相似的同名牙病例30例,自身对照方法,分别采用Er,Cr:YSGG激光(实验组)和传统涡轮牙钻(对照组)去腐备洞。使用Wong-Baker面部表情量表对治疗过程中的疼痛程度进行评估,调查每个病例对两种方法的接受程度,记录去腐备洞时间,并分别在治疗后6个月,12个月复查充填体情况。结果:和传统的涡轮牙钻去腐备洞比较,Er,Cr:YSGG激光可明显减轻备洞过程中的疼痛,差异有显著性(P<0.01);80%的患者更愿意选择Er,Cr:YSGG激光技术进行治疗;Er,Cr:YSGG激光和涡轮牙钻去腐备洞的平均时间分别是504s和390s;治疗后6个月,12个月复查,实验组和对照组差异无显著性。结论:Er,Cr:YSGG激光去腐备洞技术可显著减轻治疗过程中的疼痛和不适,在切割牙体硬组织方面具有独特的优点。

【Abstract】 Objective:To evaluate Er,Cr:YSGG laser system objectively through randomized control trial in clinic.Method:30 patient were selected randomly and the method of self-comparison was adopted.Each patient had at least two teeth of which caries degree were similar.Caries was removed and cavities were prepared using Er,Cr:YSGG laser(Biolase,USA) technique and traditional handpiece method respectively.Each patient were required to select which method he prefer through a questionnaire.The time of caries removal and cavity preparation was recorded.Patients were called back for checking restorations 6 months and 12 months after treatment.The pain degree of each patient was recorded with WongBaker Facial Pain Scale.Result:Average time of Er,Cr:YSGG laser and turbine method for were 504 s and 390s respectively.Er,Cr:YSGG laser can alleviate pain obviously compared with traditional method,the difference is significant(x2=21.99,P< 0.01).80% patients prefer Er,Cr:YSGG laser to traditional method.There is no significant difference between two group restorations 6 months and 12 months after treatment.Conclusion:Er,Cr:YSGG laser can alleviate treatment pain and discomfort obviously,so it is a new and effective technology for dental hard tissue surgery.
