【摘要】 目的探讨Graves病(GD)患者糖皮质激素诱导的肿瘤坏死因子受体(GITR)mRNA表达水平的变化及意义。方法用实时荧光定量PCR法检测22例GD未治疗组、23例GD临床缓解组及18例健康对照组外周血单个核细胞(PBMC)GITR mR-NA相对表达水平;用化学发光法测定各组血浆中甲状腺激素水平,并评价GD未治疗组GITR mRNA相对表达水平与甲状腺激素水平的相关性。结果GD未治疗组GITR mRNA相对表达水平明显低于临床缓解组和健康对照组(χ2=18.48,P<0.01;χ2=18.26,P<0.01),而GD临床缓解组与健康对照组差异无统计学意义(χ2=0.12,P>0.05)。GD未治疗组GITRmRNA相对表达水平与FT3、FT4水平均呈负相关(r=-0.509、r=-0.483,P均<0.05),而与TSH呈正相关(r=0.458,P<0.05)。结论GD的发生可能与GD患者体内GITR基因表达下调有关。

【Abstract】 Objective To explore the change and clinical significance of the expression level of glucocorticoid-induced tumor necrosis factor receptor(GITR) mRNA in peripheral blood mononuclear cells of patients with Graves disease(GD).Methods The relative expression level of GITR mRNA was detected by real-time fluorescent quantitative PCR in 22 untreated patients,23 patients with clinical remission,and 18 healthy controls.The thyroid hormone level in each group was measured by ICMA.The correlations were evaluated between the relative expression level of GITR mRNA and thyroid hormone level in untreated GD group.Results The relative expression of GITR mRNA in untreated patients was significantly lower than that in clinically resolved patients(χ2=18.48,P<0.01) and in healthy controls(χ2=18.26,P<0.01),but the difference between clinically remitted GD patients and healthy controls had no statistical significance(χ2=0.12,P>0.05).There were negative correlations between the levels of FT3 or FT4 and GITR mRNA(P<0.05),and there was a positive correlation between the levels of TSH and GITR mRNA in untreated GD group(P<0.05).Conclusions GD is associated with downregulation of GITR mRNA expression and dysfunction of Treg cells.
