非手术治疗闭合性锤状指

【摘要】 目的介绍非手术治疗闭合性锤状指的一种新方法。方法对36例闭合性锤状指患者早期应用掌侧石膏托外固定法进行治疗。石膏置于伤指远端至前臂中段掌侧,拇指与伤指对捏位固定,其中6例为远节指骨基底部撕脱骨折患者。石膏固定6周后功能锻炼。结果36例均获随访,时间4~18个月。未出现压疮。患指近侧指间关节、掌指关节和腕关节、其它手指各关节活动均正常。患指远侧指间关节伸屈活动度为0°~70°26例,0°~60°5例,0°~53°3例,0°~35°2例。将患指与健指远侧指间关节活动度相比较,疗效参照TAM系统评定方法评定:优26例,良8例,差2例,优良率为94.4%。6例撕脱骨折者3个月均骨性愈合。结论应用该石膏外固定方法治疗闭合性锤状指,操作简单,疗效确切。

【Abstract】 Objective To introduce a new method of treatment to closed mallet finger with non-surgical way.Methods 36 patients who injuried the stretching tendon′s end with closed wound were treated by early,plaster external fixation at the palm side.The plaster was placed in front of the palm side from the end of the injuried finger to the stage casing of forearm.The plaster coagulated when the thumb and injuried finger were holding.Six cases whose base of the far piece bone of finger were ripped fracture.Six weeks later,all were permitted to exercise.Results 36 cases were followed up for 4~18 months.No one got pressed wound.Motion range of patient′s injured fingers′PIP joint of finger,knuckle and wrist were normal,other fingers′motion range was normal.The motion range of the far joints of the injured fingers was:0°~70°,26 cases;0°~60°,5 cases;0°~53°,3 cases;0°~35°,2 cases.Range of motion of DIP joint of the injured fingers was compared with that of the normal one.TAM system was adopted to grade the results,26 cases of the group were excellent,8 cases were good,2 cases were poor.The excellent and good rate was 94.4%.The six ripped fracture of the far piece bone of finger healed well.Conclusions Plaster immobilization is effective and simple treat the closed mallet finger.
