河北汉族HLA-A、B、DRB1基因多态性与慢性粒细胞白血病相关性研究
Relationship between HLA-A,B,DRB1 gene polymorphism in Hebei Chinese Han and chronic myeloid leukemia
【摘要】 目的通过对河北省汉族人群进行慢性粒细胞白血病(CML)与人类白细胞抗原(HLA)基因分布频率的相关性研究,探讨HLA基因与CML发病之间的关联,及其与临床预后的相关性。方法应用序列特异性引物聚合酶链反应技术(PCR-sequence specific primers SSP)对河北省汉族人群中142例CML患者和212例正常对照进行HLA-A、B、DRB1基因的检测,比较两者之间HLA基因分布频率的差异,并通过woolf公式计算相对危险率(RR),以甄别出河北省汉族人群对CML发生有统计学意义的"易感基因"和"保护基因"。结果CML患者DRB1*04明显高于正常对照(17.4%vs 10.2%),相对危险系数(RR)=1.929>1(χ2=7.053,P=0.004),而A*02、DRB1*09在CML患者则明显低于正常对照(22.2%vs 33.4%),RR分别为(0.521,0.391),(χ2=8.956,P=0.003;χ2=9.619,P=0.002)。CML患者中A*03、B*13、DRB1*01基因频率与正常对照组相比虽有升高,且RR>1,但通过RR的显著性检验,差异无统计学意义(P>0.05)。结论DRB1*04可能为河北省汉族人群CML患者的"易感基因",而A*02、DRB1*09则可能为河北省汉族人群CML患者的"保护基因",进一步了解遗传因素在河北省汉族人群CML病因发病中的作用,对于CML的诊断、防治和预后判断开辟新的思路。
【Abstract】 Objective To investigate the correlation between chronic myeloid leukemia(CML) patients and human leukocyte antigen(HLA)-A,B,DRB1 gene in Hebei Chinese Han population.Methods HLA-A,B,DRB1 alleles were examined in 142 patients with CMLand 212 normal controls in Han individuals of Hebei Province by polymerase chain reaction-sequence specific primers(PCR-SSP) methods.The gene frequencies of HLA in CML patients were compared with those of normal controls to caculate relative risk(RR) of HLA-A,B,DRB1 alleles by Woolf formula,then to discriminate the predisposing genes and protecting genes of CML,which were of significant statistical values.Results DRB1*04 gene frequency in CML was significantly higher than that of normal controls(17.4% vs 10.2%,RR=1.929>1,χ2=7.053,P=0.004);on the contrary,A*02,DRB1*09 gene frequency in CML was significantly lower than that of normal controls(22.2% vs 33.4%,RR=0.521,χ2=8.956,P=0.003;(5.4% vs 12.6% RR=0.391,χ2=9.619,P=0.002).The HLA-A*03,B*13,DRB1 *01 were increased in CML patients compared with those of normal controls.Despite yielding a RR>1,however,the association was not statistically significant.Conclusion DRB1 *04 might be a "predisposing gene" of CML in Hebei Chinese Han population,on the contrary,A*02,DRB1*09 may play a role of protection genes in Hebei Chinese Han population.It is helpful to understanding the effective of heredity factor in the course of CML nosogenesis.It also blazes a new way in the diagnosis,prevention and prognosis of CML.
