三维超声检查对胎儿肢体畸形诊断的临床价值

【摘要】 目的探讨三维超声在胎儿肢体畸形成像中的临床价值。方法采用表面成像及透明成像方法,对15例产前常规二维超声检查发现或疑为胎儿肢体畸形的病例,进行三维超声成像检查,比较二维、三维图像提供畸形的部位及细节的准确性。结果15例均能进行三维扫查,并得出表面或最大透明图像,经产后证实二维超声对病变部位及细节判断准确者分别为12例(80.0%)和10例(66.7%)。而三维超声判断准确者分别为14例(93.3%)和13例(86.7%)。1例胎儿并指畸形二维、三维均漏诊。结论三维超声表面成像方式能够反映肢体畸形的外貌特征,采用最大透明成像方式,能够了解畸形肢体的骨骼数量和形态改变。三维超声对胎儿肢体发育异常的病变部位及细节提供更丰富的诊断信息,是二维超声的重要补充形式。

【Abstract】 Objective To explore the clinical value of three- dimensional ultrasound in diagnosis of fetal limb abnormality. Methods Fifteen pregnant women suspected or discovered as fetal limb abnormality were examined by 3D US scan.Volume - rendering imaging with surface and transparent mode was used to reconstruct 3D images.Then the accuracy of 2D sonograms for the abnormal location and details were compared with those of 3D sonograms.Results Fifteen cases were examined by 3D scanned,and got the images with surface or transparent maximum mode.After abortions,there were 12 cases of abnormal location(80.0%) and 10 cases of abnormal detail(66.7%) found by 2D US.While there were 14 cases(93.3%) and 13 cases(86.7%) found by 3D US respectively. 1 case was missed diagnosed both by 2D US and 3D US.Conclusion The surface imaging mode of 3D US can get the characteristics of abnormal limb,and the transparent maximum mode can get the bony number and shape change of abnormal limb.3D US can provide more information about the location and the detail in the diagnosis of fetal limb abnormality,it is an important complement method for 2D US.
