纤维蛋白胶/BMP-2复合物促进骨缺损早期修复的实验研究

【摘要】 目的探讨纤维蛋白胶复合骨形态发生蛋白-2(BMP-2)修复家兔桡骨缺损,扫描电镜观察新骨形成并用X线衍射能谱分析新骨形成界面钙、磷、硫元素含量的动态变化.方法将36只健康成年家兔随机分成3组,均造成双侧桡骨外侧中1/3段长8mm,宽2mm,深1mm的骨缺损.实验组:植入纤维蛋白胶与BMP-2的复合物对骨缺损进行修复;对照组:在骨缺损区单独植入纤维蛋白胶进行修复;空白组:骨缺损区不植入任何材料,骨缺损自然愈合.于术后2周、4周、8周分期处死动物,进行扫描电镜观察和X线衍射能谱分析钙、磷、硫含量.结果扫描电镜显示,实验组成骨活跃程度、骨愈合速度明显优于对照组和空白组.X线衍射能谱分析显示,随着时间的增加,新骨生成界面的Ca、P含量呈增加趋势,S含量呈减少趋势.术后第2、4周各组的Ca、P和S元素的含量差异非常显著(P<0.01),第8周差异不显著(P>0.05).结论纤维蛋白胶/BMP-2复合物能够促进早期成骨,促进骨缺损区骨组织的再生和钙化,加速骨缺损的愈合.钙、磷、硫元素的动态变化基本可反映出新骨形成界面愈合的基本进程.

【Abstract】 Objective To investigate the effect of the fibrin glue (FG) as a carrier compounded with BMP-2 on repairing bone defect and stimulating early period bone formation and to provide the experimental foundation for the clinical application.Methods Bone defects (8.00 mm length,2.00 mm width and 1.00 mm depth) were created in 36 rabbits on the bilateral radius and were treated with three kinds of implantations. Experimental group:fibrin compounded with BMP-2;control group:fibrin;blank control group:no material. The animals were divided to 3 groups,then the defects were repaired with three materials above.Animals were respectively sacrificed at the end of 2 weeks,4 weeks and 8 weeks after operation.Then the speciments were made for scanning electric microscope observation and energy dispersive analysis X-ray (EDAX) of element Calcium (Ca),Phosphonium (P) and Sulphur (S).Results The bone forming activity and bone healing speed in experimental group were significantly better than those in the other two groups by scanning electric microscope observation.The result of EDAX showed that the content of element Ca and P of the newly formed bone increased obviously through the second and fourth week (P<0.01),while the contents of element S decreased obviously (P<0.01).After the eighth week,there was no obvious difference in the amount of three elements of newly formed bone between the experimental group and other groups (P>0.05). Conclusions The material with fibrin glue as a carrier compounded with BMP-2 can significantly accelerate the new bone forming,and it is beneficial for regeneration,mineralization and remodeling.It can stimulate early bone formation and osseointegr-ation.This study indicates that the concentration changes of Ca,P and S correlate with the processes of interface mineralization.
