【摘要】 目的探讨引起药物性肝病的常见药物、发病规律、临床特点及治疗转归.方法对179例因药物性肝病住院患者的临床资料进行回顾性研究.结果引起肝病的药物种类繁多,以各类中药为主,占44.2%,其次为抗生素,共占24.6%;其中抗结核药物占13.4%;潜伏期大多数为1～8周(64.8%),但不少病例(26.8%)用药至发现肝损伤时间>12周.临床类型主要为胆汁淤积型(57.5%),重型肝炎少见;主要表现有乏力(54.8%)、纳差(61.5%)、黄疸(65.4%).结论引起药物性肝病病因中以中药为主,临床分型主要为胆汁淤积型,详细询问用药史和用药过程中监测肝功能是诊断的关键.应积极建立我国药物性肝病的数据库.更多还原

【Abstract】 Objective To analyze drug-induced liver disease case so as to explore its common pathogeny,occurrence regularity,clinical characteristics and therapeutic outcomes.Methods 179 cases of inpatients with drug-induced liver disease were investigated to analyze the correlated factors.Results The species caused drug-induced liver disease were various. Traditional Chinese Medicine stood out as the first drug related to drug-induced liver disease(44.2%). The second cause was antibiotics(24.6%),including antituberculous drugs(13.4%).Most cases’incubation period was 1～8 weeks(64.8%),while for a few cases(26.8%),the incubation period of liver function injury after using drugs exceeded 12 weeks.The cholestasis pattern of damage was the most common(57.5%).Gravis type of hepatitis was few.The main clinical manifestations included debility(54.8%),decreased appetite(61.5%),jaundice(65.4%).Conclusions The commonest cause of drug-induced liver disease is Traditional Chinese Medicine.Its main clinical type is the cholestasis one.Detailed asking medication usage and monitoring liver function before and after the period of taking drugs are the two key factors of diagnosing drug-induced liver disease.We should actively set up databases about drug-induced liver disease.更多还原
