下呼吸道感染患者中分离鲍曼不动杆菌的耐药性分析

【摘要】 目的了解鲍曼不动杆菌医院下呼吸道感染的分布及耐药状况,为临床合理用药提供依据。方法收集我院2005年1月—2007年12月住院患者下呼吸道感染的356株鲍曼不动杆菌,采用纸片扩散法测定菌株对抗菌药物的敏感性,数据用WHONET5.0及SPSS13.0软件进行统计分析。结果鲍曼不动杆菌感染以下呼吸道感染为主,临床分离率逐年升高,多重耐药菌株占77.2%,主要分布于ICU。药敏试验结果显示该菌对亚胺培南和美罗培南敏感率最高,分别为76.7%和74.2%,其次为头孢哌酮-舒巴坦67.1%,对其他所测抗菌药物的敏感率均小于30%。碳青霉烯类抗生素耐药的菌株除头孢哌酮-舒巴坦外,对常用抗菌药物的耐药率超过80%。结论鲍曼不动杆菌多重耐药严重,集中分布于ICU。碳青霉烯类抗生素仍是敏感性最高的药物,头孢哌酮-舒巴坦次之,其他抗菌药物耐药率高,宜根据药敏试验结果选用。

【Abstract】 Objective To investigate the prevalence and antibiotic resistance of A.baumannii in lower respiratory infections for the rational use of antimicrobial agents.MethodsA total of 356 A.baumannii isolates were collected from January 2005 to December 2007.The antimicrobial susceptibility was analyzed by disk diffusion method.All the data were analyzed by WHONET 5.0 and SPSS 13.0 software.ResultsA.baumannii were mainly from lower respiratory tract.The prevalence of A.baumannii appeared to increase year by year.About 77.2% of the isolates were multiple resistant.These strains were mainly isolated from ICUs(70.8%,252/356).The antibiotic susceptibility testing showed that imipenem(76.7% susceptible)and meropenem(74.2% susceptible)were the most active agents against A.baumannii,followed by cefoperazone-sulbactam(67.1% susceptible).The susceptibility to other antimicrobial agents was lower than 30%.ConclusionsA.baumannii strains are highly resistant to multiple antibiotics.These strains are mainly isolated from ICU.Carbapenems are still the most active agent against A.baumannii,followed by cefoperazone-sulbactam.A.baumannii is highly resistant to other antimicrobial agents.Therapy should be decided according to the results of susceptibility testing.
