【摘要】 目的探讨阿尔茨海默病(AD)行为和精神症状(BPSD)的特点及其相关因素。方法对符合美国精神障碍诊断和统计手册第四版修正版(DSM-IV-R)诊断标准的46例住院AD患者,进行一般资料调查,评定简易智力状态检查(MMSE)、临床痴呆评定表(CDR)、AD病理行为评分表(BE-HAVE-AD)及日常生活能力量表(ADL),分析BPSD症状特点及与有关因素的关系。结果BPSD的发生率为100%,其中以行为紊乱发生率最高(91.3%)。幻觉发生率以重度AD最高(47.6%),MMSE分值与BEHAVE-AD幻觉分量表分呈负相关(P<0.05)。患者年龄与BEHAVE-AD行为紊乱、日夜节律紊乱分量表分和总分呈负相关(P<0.05或P<0.01)。ADL分值与BEHAVE-AD幻觉、行为紊乱、日夜节律紊乱分量表分及总分呈正相关(P<0.05)。结论住院AD患者BPSD的发生率非常高,应引起临床足够的重视。幻觉的出现可作为AD痴呆严重度的预警因素。AD患者BPSD可能与年龄和日常生活能力有关。更多还原

【Abstract】 Objective:To explore the features and relative factors of behavioral and psychological symptoms of dementia (BPSD) in patients with Alzheimer’s disease(AD). Methods:Forty-six hospitalized patients with AD according to the Diagnostic and Statistical Manual of Mental Disorders,4th edition-revised (DSM-IV-R)were assessed with the general information questionnaire,Mini Mental State Examination (MMSE),Clinic Dementia Rating (CDR),the Behavioral Pathology in Alzheimer’s Disease Rating Scale (BEHAVE-AD) and Activity of Daily Living Scale (ADL). The features and the relative factors of BPSD were analyzed. Results:The prevalence of BPSD was 100%,among which the behavioral disturbance was the most common(91.3%)symptom. Hallucination was the most common symptom in patients with the severe AD(47.6%). The score of and MMSE was negatively correlated with the score of the subscale of hallucination in BEHAVE-AD (P<0.05). The was negative correlation between age and the scores of the subscales of behavioral disturbance and day-night rhythm disturbance and the total score of BEHAVE-AD (P<0.05 or P<0.01). The score of ADL was positively correlated with the scores of the subscales of hallucination,activity disturbance,day-night rhythm disturbance and the total score of BEHAVE-AD (P<0.05). Conclusion:The behavioral and psychological symptoms are common in hospitalized patients with AD and should be paid more attention. Hallucination may be the sign of the severe AD. Age and activity of daily life are correlated with BPSD in AD.更多还原
