血液灌流联合CVVH治疗在MODS中的治疗作用
Effects of hemoperfusion plus continuous venous-venous hemofiltration for patients with multiple organ dysfunction syndrome
【摘要】 目的:观察血液灌流(hemoperfusion,HP)联合持续-静脉血液滤过(Continuous Venous-venous Hemofiltration,CVVH)对多器官功能障碍综合征(multiple organ dysfunction syndrome,MODS)患者的临床疗效及细胞因子的影响,探讨HP联合CVVH对MODS患者的治疗作用。方法:20例行HP联合CVVH的MODS患者为研究对象,比较他们治疗前、后生命体征、白细胞、氧分压、肝、肾功能和细胞因子TNF-α、IL-1β、IL-8水平的变化。结果:HP联合CVVH治疗后的MODS患者其临床指标与治疗前比较明显改善,细胞因子TNF-α、IL-1β、IL-8的水平与治疗前比较明显降低,差异有统计学意义(P<0.05)。结论:HP联合CVVH能改善MODS患者的临床指标,降低MODS患者细胞因子水平,可改善MODS患者的预后。
【Abstract】 ObjectiveTo observe the effects of hemoperfusion(HP) plus continuous venous-venous hemofiltration(CVVH) on the vital sign,white blood cell,partial pressure of oxygen,hepatic function,renal function and cytokines in patients with multiple organ dysfunction syndrome(MODS) and to investigate the therapeutic action of HP + CVVH for patients with MODS.MethodTwenty patients with MODS underwent HP+ CVVH were brought into trial.The vital sign,white blood cell,partial pressure of oxygen,hepatic function,renal function and the levels of TNF-α,IL-1β,IL-8 in patients with MODS were measured before HP+ CVVH and after HP+ CVVH respectively.ResultsThe vital sign,white blood cell,partial pressure of oxygen,hepatic function and renal function were improved after HP+ CVVH treatment.The levels of TNF-α,IL-1β and IL-8 in patients with MODS after HP+ CVVH were lower than those before HP+ CVVH(P<0.05).ConclusionHP+ CVVH could improve clinical picture and decrease the levels of cytokines in patients with MODS,and ameliorate the survival rate of MODS patients.
