脑梗死并发下肢深静脉血栓形成50例临床分析

【摘要】 目的探讨脑梗死(cerebral infarction,CI)并发下肢深静脉血栓形成(deep veins thrombosis in lower limb,LD-VT)患者的临床表现、诊断、治疗及预防对策。方法本文对50例CI后LDVT患者的原始资料进行回顾性分析,分析一般状况、临床特点以及静脉尿激酶溶栓或低分子肝素抗凝治疗效果。结果CI病人并发LDVT患者中,65岁以上66%;下肢水肿伴疼痛为主要症状,瘫痪侧LDVT发生率82%,健侧LDVT发生率14%,双侧LDVT发生率4%,通过静脉尿激酶溶栓或低分子肝素抗凝治疗,7~14d内所有深静脉血栓的症状逐渐消失。结论静脉壁损伤、血流缓慢和高凝状态是CI患者发生LDVT的因素,预防卒中患者LDVT的发生是降低CI致残率及病死率的关键。彩色多普勒超声是检测LDVT的一种理想工具,对发现及预防LDVT有重要意义。溶栓与抗凝疗法是治疗深静脉血栓的有效手段。

【Abstract】 Objective To analyze 50 cases of cerebral infarction (CI) patients combined with deep venous thrombosis in lower limb (LDVT) and the syndromes,treatment and prevention. Methods In a retrospective study of 50 LDVT patients after cerebral infarction,general conditions ,clinical characteristics and the effective treatment (low-molecular-weight heparin or urokinase)were retrospectively analyzed.Results The incidence of LDVT in elderly CI patients (above 65 years old) was 66%. Pain and swelling were the main symptoms. In the 50 patients,LDVT incidences at paralytic side,normal side and both sides were 82%,14% and 4%,respectively. After the treatment ,the syndromes of LDVT disappeared gradually in 7 to 14 days. Conclusion Injury to the blood vessel wall,reduced or stagnant blood flow and hypercoagulability are three principal factors of LDVT. Preventing cerebral infarction patient from LDVT is one crucial work to decrease the mortality and disability rate in cerebral infarction patients. Color Doppler ultrasound is an ideal method for LDVT diagnosis. Thrombolysis treatment and anticoagulant treatment is effective for LDVT.
