高血压性脑出血患者血肿周围脑组织ICAM-1的表达及意义

【摘要】 目的探讨脑出血后血肿周围组织细胞间黏附因子-1(Intercellular Adhesion Molecule-1,ICAM-1)的表达与脑水肿的关系。方法剖解39例脑出血后不同时间死亡病人的脑组织,自出血灶边缘向外1.0~3.0 cm及出血灶对侧相应部位的脑组织进行取材,出血灶对侧设为对照组。采用HE染色、免疫组化技术观察不同时间点出血灶周围ICAM-1在脑组织中的表达和变化规律。结果脑出血后2 h出血灶周围血管和神经细胞,即有ICAM-1的表达(2.12±0.32)个/高倍视野,17~24 h血肿周围ICAM-1阳性微血管数开始明显增加(6.02±1.05)个/高倍视野,72 h达到高峰(11.10±0.87)个/高倍视野,168~312 h后逐渐减弱(4.09±0.56)/高倍视野。此外,对侧脑实质内可见阳性神经细胞及少量ICAM-1阳性微血管。结论脑出血后,出血灶周围ICAM-1的表达增加,对脑出血后脑水肿的形成起着重要的促进作用。

【Abstract】 Objective To investigate the relationship between the expression of Intercellular Adhesion Molecule-1(ICAM-1) in the perihematomal tissues and the brain edema in human hypertensive intracerebral hemorrhage(ICH).Methods Paraffin-embedded brain tissues of 39 fatal cases ofhuman ICH were stained with HE and immunohistochemistry staining.The tissues were obtained from the perihematomal tissues 1.0~3.0 cm and the opposite regional tissues were used as control group.The expression changes of ICAM-1 in the perihematomal tissues were analysised by SPSS 11.5 software.Results The ICAM-1 expressed in capillaries and nerve-cells in the perihematomal tissues after 2 h of ICH(2.12±0.32/HP).The ICAM-1 positive microvessel count increased significantly at 17~24 h after ICH(6.02±1.05/HP),peaked at 72 h(11.10±0.87/HP) and dropped gradually at 168~312 h(4.09±0.56/HP).In addition,There were little ICAM-1 positive nerve-cells and capillaries in the opposite brain tissues.Conclusion The increased expression of ICAM-1 after ICH was correlated with the brain edema formation in the perihematomal tissues.
