【摘要】 目的比较等效剂量的舒芬太尼与芬太尼在腹腔镜卵巢瘤切除术的麻醉效果。方法 40例腹腔镜卵巢瘤切除患者随机分为舒芬太尼（sufentanil,Suf）组和芬太尼（fentanyl,Fen）组,术中连续监测血流动力学,分别于麻醉前（T₀）、插管后1 min （T₁）、气腹后5 min（T₂）、拔管即刻（T₃）记录收缩压（systolic blood pressure,SBP）、舒张压（disastolic blood pressure,DBP）、平均动脉压（mean arterial pressure,MAP）、心率（heart rate,HR）并计算心率收缩压乘积（rate pressure product,RPP）.记录清醒时间、拔管时间,并对术后疼痛进行视觉模糊评分（visualanalogue pain scale,VAS）.结果 Fen 组 T₁、T₂、T₃时点 SBP、DBP、MAP、RPP 均高于 T₀,且均高于 Suf 组。Fen组 T₁时点 HR 高于 T₀,且高于 Suf 组（P<0.05）。Suf 组血流动力学更稳定;意识恢复时间、气管拔管时间均明显短于 Fen 组,而且苏醒期患者恢复平顺,少有躁动、恶心、呕吐等;术后疼痛 VAS 评分明显小于 Fen 组.结论舒芬太尼应用于卵巢瘤切除术血流动力学稳定,镇痛效果好,副作用少,安全有效。更多还原

【Abstract】 Objective To compare the anesthetic effect of the equivalent dose of sufentanil and fentanyl in laparoscopic ovarian tumor resection.Methods Forty patients undergoing laparoscopic ovarian tumor resection were randomly divided into sufentanil （ Suf） group and fentanyl （Fen） group.Continuously monitor the hemodynamic,note systolic blood pressure （SBP）,disastolic blood pressure（DBP）,mean arterial pressure（ MAP）,heart rate（ HR） and rate pressure product（RPP） of two groups before using revulsant（T₀）,1 min after intubation （T₁）,5 mins after pneumoperitone um （T₂ ） and the moment of extubation （T₃）.Record the awake time and extubation time,and evaluate the visual analogue pain scale（ VAS） score of postoperation. Results SBP,DBP,MAP,RPP of Fen group at T₁,T₂,T₃ moment were higher than those of T₀ moment and Suf group.HR of Fen group at T₁ moment was higher than that of T₀ moment and Suf group.Hemodynamic of Suf group was more stable,awake time and extubation time were significantly shorter than Fen group,patients in Suf group regained consiousness smoothly with little restlessness,nausea,vomiting.Postoperative pain VAS score was significantly less than Fen group.Conclusion Sufentanil was a safe and effective anethetic analgesic for laparoscopic ovarian tumor resection with stable hemodvnamics and fewer side effects.更多还原
