【摘要】 目的探讨保留悬雍垂腭咽成形术(H-UPPP)治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的效果。方法应用H-UPPP治疗睡眠呼吸暂停综合征患者39例,男33例,女6例。根据术前和术后6个月多导睡眠监测(PSG)结果,评估手术疗效;用问卷调查方式评价患者术后并发症的发生率及主观症状的改善程度。结果39例OSAHS患者术前行PSG监测结果示:轻度6例,中度24例,重度9例;术后6个月复查PSG结果示:治愈5例(12.82%);显效11例(28.21%);有效13例(33.33%);无效10例(25.64%);总有效率为74.36%。32例(82.05%)患者术后打鼾、嗜睡等症状明显改善,7例(17.95%)患者症状轻度改善。结论应用H-UPPP手术治疗OSAHS,手术简便安全,治愈率高,并发症少,该术式值得临床推广应用。更多还原

【Abstract】 Objective To investigate the curative effect of H-uvulopalatopharyngoplasty(H-UPPP) on obstructive sleep apnea hypopnea syndrome(OSAHS).Methods The improved H-UPPP operation was performed on 39 patients who were 33 men cases and 6 women cases with the OSAHS.The surgical effects which including the rate of the postoperative complication and the improvement of subjective symptoms for the patients were evaluated by questionnaires and polysomnography(PSG) before and 6 months after H-UPPP.Results PSG showed that the total of the response effective rate amounted to 74.36 % on the 6th month postoperatively.Among the 39 cases,5 cases were cured which cure proportion was 12.82%,11 cases were showed significant effect which the proportion was 28.21 %,13 cases got evident effectiveness which was 33.33 %,and 10 cases did not obtain treatment effect which was 25.64%.Based on the questionnaire investigation in 6 months,32 cases symptoms of snore,breath holding,headache and lethargy reduced obviously which amounted to 82.05%,and 7 cases symptoms of those reduced slightly which amounted to 17.95%.Conclusion The results about the surgery of the uvula preserving H-UPPP indicate that it is the simple,safety,and higher cure rate,and lower postoperative complications compliment.The surgery may be popularized application in the hospital.更多还原
