宫颈病变妇女人乳头瘤病毒感染模式和基因型分布研究

【摘要】 目的研究宫颈病变妇女的人乳头瘤病毒感染模式及基因型分布状况。方法利用PCR-RDB法检测妇女宫颈上皮细胞人乳头瘤病毒感染情况及基因型,TCT技术检测其宫颈病变状况。结果NILM、ASCUS、LSIL、HSIL组的HPV感染率分别为28.81%、68.63%、80.95%和94.12%,4组间差异有统计学意义(x~2=76.23,P<0.01)。单一基因型人乳头瘤病毒感染与多种基因型合并感染比例分别占73.86%(113/153)和24.18%(37/153),4组间差异无统计学意义(x~2=5.70,P=0.127)。HPV16、HPV52和HPV58型为最常见的3种感染类型,其所占比例分别为19.17%(37/193)、17.62%(34/193)和13.47%(26/193)。HSIL组中,HPV16型所占比例最高,与其他3组比较,差异有统计学意义(x~2=0.042,P<0.05)。结论宫颈病变与人乳头瘤病毒感染有关;人乳头瘤病毒感染模式以单一基因型感染为主,合并感染为辅;HPV16为最常见感染类型,其次为HPV52和HPV58型。

【Abstract】 Objective To study the infectious mode and genotypes of human papillomavirus (HPV) in women with cervical lesion.Methods The HPV infection status and genotypes of cervix epithelial cells were detected with polymerase chain reaction/revert dot blotting(PCR/RDB),and thinprep cytologic test(TCT) was applied to detecting cervical lesion.Results The HPV infection rate was 28.81%,68.63%,80.95%and 94.12%respectively in intraepithelial lesion or malignancy (NILM) group,atypical squamous cell of unde-termined significance(ASCUS) group,low-grade squamous intraepithelial lesion(LSIL) group,and high-grade squamous intraepithelial lesion(HSIL) group.Significant differences were found among them(x~2=76.23,P<0.01).The rate of single genotype infection or multiple genotypes infection of NILM,ASCUS,LSIL and HSIL were 73.86% (113/153) and 24.18%(37/153) respectively;there were no statistical differences among the four groups(x~2=5.70,P=0.127>0.05).HPV 16,HPV52 and HPV58 were the most common HPV genotypes,accounting for 19.17%(37/193),17.62%(34/193) and 13.47%(26/193),respectively. The rate of HPV16 genotype was the highest in HSIL group,significantly higher than that of the other three groups(x~2=0.042,P<0.05).Conclusion The HPV infection rate is increased with severity of the pathological changes.Single genotype infection is predominant;HPV16 is the most common genotype,followed by HPV52 and HPV58.
