重度颅脑损伤患者继发心肌损害的临床分析
Clinical Study on Cardiac Muscle Lesion Secondary to Serious Brain Injury
【摘要】 目的:探讨重型颅脑损伤患者血清肌钙蛋白I(CTnI)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)浓度和心电图的变化,为重型颅脑损伤继发心肌损害患者的病情观察和预后评估提供定量指标。方法:以34例健康成人为对照,检测45例重型颅脑损伤患者入院时的血清CTnI、CK、CK-MB浓度,并进行心电监测。结果:颅脑损伤患者血清CTnI、CK、CK-MB浓度较对照组明显增高,并且浓度的增高与预后间的关系有统计学意义。45例患者中有32例出现了心电图的异常,心电图的异常率与病情和预后相一致。结论:血清CTnI、CK、CK-MB浓度的测定和心电监测有助于重型颅脑损伤继发心肌损害患者病情的观察和预后的评估。
【Abstract】 Objective To detect the relationship between elevation of the serum cardiac injury markers and severlity of the disease process in patients with serious brain injury.Methods Serum concentrations of CTnI(with ELISA) CK(with biochemistry) and CK-MB(with immuno-inhibition method) were determined in 45 cases of serious brain injury at admission with continuous EKG monitoring as well as in 34 controls.Results The serum concentrations of CTnI,CK and CK-MB in the patients were significantly higher than those in the controls.The higher the concentrations were,the worse the prognosis was.Among the 45 cases,32 patients had abnormal electrocardiogram.The abnormality of electrocardiogram was correlated with the seriousness of the condition and prognosis.Conclusion The detection of serum CTnI,CK,CK-MB levels changes and electrocardiogram monitoring were helpful for assessment of progress of disease and prognosis in patients suffering from cardiac muscle lesion secondary to serious brain injury.
