吸入糖皮质激素对支气管哮喘婴幼儿胰岛素样生长因子、骨钙素及瘦素的影响

【摘要】 【目的】探讨长期吸入糖皮质激素对哮喘婴幼儿胰岛素样生长因子及骨钙素、瘦素的影响,评价吸入糖皮质激素对哮喘婴幼儿的有效性、安全性及对生长发育的影响。【方法】支气管哮喘婴幼儿72例(观察组)给予氟替卡松吸入治疗。于治疗前、治疗6、12、24个月采静脉血5 mL,用酶联免疫吸附法(ELISA)测定胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)及胰岛素样生长因子结合蛋白-3(insulin-like growth factor binding protein-3,IGFBP-3);放射免疫分析法测定血清瘦素(leptin,LEP)和骨钙素(osteocalcin,BGP)水平。应用标准法测量并记录二组身高、体重。【结果】观察组婴幼儿用药前及用药6、12、24个月后其血清GF-1I、GFBP-3、LEP及BGP的水平比较差异无显著性(P均>0.05);观察组身高、体重治疗前、治疗后24个月时与正常年龄和匹配健康儿童比较差异无显著性(P均>0.05)。【结论】长期吸入糖皮质激素对哮喘婴幼儿血清IGF-1、IGFBP-3及LEP、BGP及生长发育无明显影响,对哮喘婴幼儿是有效、安全可靠的。

【Abstract】 【Objective】To explore the changes of insulin like growth factor system,osteocalcin and leptin before and after long-term inhaled corticosteroids in infants with asthma and to observe the variation of height and weight between sick infants and healthy infants with same age and same gender before and after 24 months therapy respectively. 【Methods】Seventy-two infants with asthma were collected as observation group and patients were received long-term inhaled cortices steroids.The height and weight was measured and recorded by standard method;the serum blood were collected before treatment and after treatment 6,12,24 months,the contents of osteocalcin(BGP) and leptin(LEP) were measured by radio immunoassay(RIA).The serum insulin-like growth factor-1(IGF-1) and insulin-like growth factor binding protein-3(IGFBP-3) level were measured by enzyme-linked immunosorbentassay(ELISA). 【Results】The levels of serum IGF-1,IGFBP-3,BGP and LEP in infants with asthma had no significant differences before and after 6,12,24 months therapy(P>0.05).The height and weight of infants in observation group and control group which were healthy and had the same age and gender had no significant differences before therapy(P>0.05) and after 24 months therapy(P>0.05). 【Conclusions】The serum concentrations of IGF-1,IGFBP-3,BGP and LEP in infants suffered from asthma have no obvious change before and after 24 months long-term inhaled corticosteroids.The height and weight changes before and after therapy have no significant difference between observation group and control group with same age and gender.
