固尔苏及鼻塞间歇正压通气治疗新生儿严重呼吸窘迫综合症

【摘要】 【目的】评价鼻塞间歇正压通气(intermittent positive-pressure ventilation,IPPV)及肺表面活性物质(固尔苏)对新生儿严重呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)的治疗作用,并与常规IPPV比较。【方法】18例患RDS的新生儿(III级和IV级)经气管内给予固尔苏[(100 mg/(kg.dose)]后,改鼻塞IPPV进行治疗,另25例RDS患儿给予同样剂量固尔苏后,予常规IPPV治疗,比较两组患儿血气变化,肺部感染,肺出血,气胸及慢性肺病的发生率,吸氧及住院时间和治愈率。【结果】治疗后6,12,24 h,两组患儿的血气均明显改善,两组比较,差异无显著性。而鼻塞IPPV组患儿的肺部感染及慢性肺病的发生率明显低于常规IPPV组,吸氧时间也少于常规IPPV组。两组的住院时间、肺出血及气胸的发生率无明显差异。【结论】鼻塞IPPV及固尔苏能有效地治疗严重地NRDS,与常规IPPV比较,具有肺部感染及慢性肺病发生率低,氧疗时间少等优点。

【Abstract】 【Objective】To evaluate the effect of nasal intermittent positive-pressure ventilation(IPPV) combined surfactant on neonates with repiratory distress syndrome and compare the effect with conventional IPPV. 【Methods】43 babies with severe respiratory syndrome were given curosurf [(100 mg/(kg·dose)],then extubate to nasal IPPV.Another 25 babies continued on conventional IPPV.Blood gase,rates of respiratory infection and chronic lung disease,durations in oxygen and hospitalization were compared between two groups. 【Results】Arterial partial pressure of oxygen increased,arterial pressure of carbondioxide and oxygen index decreased significantly by the time of 6,12,24 hours after treatment in both groups.There were not significantly difference between two groups.The rates of respiratory infection and chronic lung disease in nasal IPPV group were less than that of conventional ventilation group,and the duration in oxygen was also less in nasal IPPV group than that in conventional convention group,and there was significant difference between two groups.But there was not significant difference in duration of hospitalization between two groups. 【Conclusion】Surfactant combined nasal IPPV is a potential efficient therapy to the babies with severe repiratory distress syndrome,and it have lower rates of lung infection and chronic lung disease,fewer duration in oxygen compared with conventional ventilation.
