【摘要】 目的了解我院2008年临床分离大肠埃希菌对各类抗生素的耐药性。方法收集2008年我院住院患者大肠埃希菌临床分离株,按统一的方法和判断标准(CLSI2005年版)进行大肠埃希菌的药敏试验和超广谱β内酰胺酶(ES-BLs)检测的确认试验。结果283株大肠埃希菌临床分离株中,ESBLs阳性的为42.05%,ESBLs阴性的为57.95%。产ESBLs大肠埃希菌对青霉素类、头孢菌素类、单酰胺环类、氟喹诺酮类、氨基糖苷类中的庆大霉素和妥布霉素的耐药率明显高于非产ESBLs大肠埃希菌,对哌拉西林/他唑巴坦、呋喃妥因的敏感率较高,未发现对亚胺培南、美洛培南及厄他培南耐药菌株。结论为应对日趋严重的产ESBLs大肠埃希菌对抗生素的耐药趋势,应加强对抗菌素应用的规范化管理,在医院内进行大肠埃希菌耐药性监测,对了解其耐药性变迁和指导临床合理用药有很大的参考价值。更多还原

【Abstract】 Objective To investigate the antibiotic resistance of Escherichia coli collected in our hospital in 2008.Methods All Escherichia coli strains were isoloted from varies departments during the period from Jan.2008 to Dec.2008 in our hospital.Susceptibility testing was carried out by means of a unified protocal method and the results were analyzed according to CLSI 2005 Standard.At the same time the affirmed test of the ESBLs was carried out.Results Of the total 283 clinical isolates of E.coil,42.05% were ESBLs positive,57.95% were ESBLs negative.The antirnicrobile resistance of the ESBLs positive E.coli was higher than the ESBLs negative E.coli,including penicillins,cephalosporins,aztreonam,fluoroquinolones,gentamicin,tobramycin,etc.The sensitivity to piperacillin-tazobactam sodium and nitrofurantoin is high.It was not found the resistance of imipenem,meropenem or ertapenem.Conclusion To coping with the gradually severe antirnicrobile resistance of the ESBLs positive E.coil,we must strengthen administrating the reasonable usage of antimicrobile agent and the detection of resistance.It helps to comprehend the transition of the resistance and guide the reasonable usage of antimicrobile agent.更多还原
