【摘要】 目的:探讨腰麻-硬膜外阻滞联合麻醉用于老年患者髋部手术的安全性。方法:66例65岁以上老年患者髋部手术实施腰-硬联合麻醉,比较入室时与腰麻起效时,麻醉阻滞完善时循环呼吸功能的变化。结果:腰麻起效时患者SBP,DBP,MBP,HR,RPP均有下降,有显著性差异(P<0.05),经处理后均恢复。ECG,RR,SpO2,CVP无统计学意义(P>0.05)。术中恶心、呕吐2例。无术后并发症发生。结论:腰-硬联合麻醉可在老年患者髋部手术中安全应用。但要注意液体补充,控制麻醉平面,观察循环呼吸功能变化,及时处理发生的并发症。更多还原

【Abstract】 Objective: To explore the safety of lumbar-epidural-combined anesthesia applied in the hip operation over aged patients.Methods:the lumbar-epidural-combined anesthesia was applied in the hip operation over 66 patients who were more than 65 years old.The changes of respiratory function were compared among the three periods,when the patient entered the operation room,when lumbar anesthesia became effective and when block anesthesia came into effect.Result: when lumbar anesthesia became effective,the patient’s SBP,DBP,MBP,HR,RPP decreased and have remarkable difference(P<0.05) and they recovered after treatment.ECG,RR,SpO2,CVP have no statistical significance(P>0.05).In two cases the patient had nausea and emesia during the operation.After operation no complication took place.Conclusion: The lumbar-epidural-combined anesthesia can be safely applied in the hip operation over aged patients.But we should supplement liquid,control the level of anesthesia,observe the change of respiratory function and deal with complication in time.更多还原
