精神分裂症患者CYP2C19、CYP2C9的基因多态性与氯氮平血药浓度的关联分析

【摘要】 目的:探讨CYP2C9和CYP2C19基因多态性与氯氮平代谢个体差异之间的相关性。方法:应用PCR与DNA测序相结合的方法检测精神分裂症患者CYP2C19、CYP2C9基因多态性。应用高效液相色谱测定氯氮平的血药浓度,以阳性和阴性症状量表(positiveand negatve symptom scale,PANSS)评分减分率评价药物临床疗效。基因型和等位基因频率分布的两组间比较采用x~2检验。组间血药浓度和疗效比较采用ANOVA检验。结果:91例氯氮平治疗的精神分裂症患者,氯氮平血药浓度与CYP2C9*3、CYP2C19*2、CYP2C19*3基因多态性之间没有相关性(P>0.05)。结论:CYP2C9*3、CYP2C19*2和CYP2C19*3基因多态性与氯氮平血药浓度之间不存在基因剂量关系。

【Abstract】 Objective: To explore the correlation between CYP2C9, CYP2C19 genetic polymorphisms and metabolism of clozapine on individual with schizophrenia. Methods: Polymerase chain reaction (PCR) and DNA sequencing were used to examine CYP2C19 and CYP2C9 genetic polymorphisms of schizophrenics. Using high-performance liquid chromatography to measure the plasma concentrations of clozapine respectively. The clinical effectiveness was evaluated by the reduction of the positive and negative symptom scale (PANSS). Differences of genotype and allele frequency distributions of these polymorphisms were compared using x2test and the differences in plasma concentration and clinical effectiveness among genotype groups were tested by the Analysis of Variance (ANOVA). Results: There is no significant correlation between the CYP2C9*3、CYP2C19*2、CYP2C19*3 genotypes and the plasma concentration of clozapine among 91 patients. Conclusions:There is not a tendency of effect in the gene-desage of CYP2-C9*3、CYP2C19*2 and CYP2C19*3 genetic polymorphisms on clozapine plasma concentration.
