临床病原菌的流行特点及耐药性分析研究

【摘要】 目的:为了解院内病原菌的流行分布及其耐药状况,指导临床合理用药和控制院内感染。方法:分析肿瘤患者医院感染病例,采用WHONET5. 3软件进行统计分析。结果:病原菌检出率,G-杆菌占首位59.9%,其次是G~+球菌占22.7%,真菌位居第三位占17.4%。病原菌感染位居前几位的依次是大肠埃希菌、克雷伯菌、铜绿假单胞菌、凝固酶阴性葡萄球菌、白色念珠菌、金黄色葡萄球菌。耐药菌株检测发现MRSA检出率为40.7% ,MRCNS为50.0%,未检出VRS株,万古霉素、替考拉宁对革兰氏阳性球菌的敏感性最好;大肠埃希菌、克雷伯菌和奇异变形杆菌中产ESBIs的菌株检出率分别为68.2%、26.2%和10.0%,亚胺培南、美罗培南、三代头孢+酶抑制剂、阿米卡星对革兰氏阴性杆菌有较高敏感性。结论:细菌的多重耐药是我院目前面临的重要问题,在临床上应改进治疗手段,重视细菌培养和合理用药,尽量减少细菌耐药性的发生。

【Abstract】 Objective:To know the popular distribution and drug resistance analysis of clinical pathogens to provide the gist for clinical therapy and control the direction of nosocomial infection.Methods:Analyse the cases of tumour patients suffer from nosocomial infection,WHO- NET5.3 was applied for analysis.Results:Pathogens checkout rate:Gram-negative bacilli head the list was 59.5 %,Gram-positive cocci was 22. 7%,epiphyte was 17.4%.The most common species Pathogens infection were Escherichia coli,Klebsiella pneumoniae,Pseudomonas aerugino- sa,coagulase negative Staphylococcus,Candida.albicans,Staphylococcus aureus.The survey of drug resisitance strains discovered that MRSA checkout rate was 40.7%,MRCNS checkout rate was 50.0%.The VRS strains was not discovered.the suaceptibility rate to vancomycin and mincocycline were the best;The rates of producing ESBLs strains of Escherichia coli,Klebsiella pneumoniae,Proteus.mirabilis were 68.2%,26. 2% and 10.0%.The susceptibility of Gram-negative bacilli to imipenem,meropenem,era cephalosporin and enzyme inhibitor,amikacin,was higher.Conclusion:Resistance of drug was an important question in our hospital,we should improve therapy artifice,and pay attention to bacte- rial cultivate,use drugs in reason,to reduce the rate of drug resistance as possible as we could.
