【摘要】 目的探讨经支气管动脉介入治疗中晚期肺癌的临床疗效。方法50例经支气管镜或经皮肺穿刺活检病理确诊的肺癌患者,采用Seldinger技术行选择性支气管动脉插管施行支气管动脉灌注化疗栓塞术。根据肿瘤的病理类型选择支气管动脉的灌注化疗药物,栓塞剂使用超液化碘油及明胶海绵。结果治疗后复查进行疗效评价,CR4例(8%),PR26例(52%),SD14例(26%),PD6例(12%)。显效(CR+PR)30例(60%),有效(CR+PR+S)44例(88%)。经治疗后,除6例无效外,其余病例咳嗽、胸痛、气急、咯血等症状均好转或消失,其中胸水减少6例,肺不张复张10例,阻塞性肺炎完全或部分吸收12例。结论超选择支气管动脉介入治疗是治疗中晚期肺癌的一种新的,有效的方法,可有效缓解患者的临床症状。更多还原

【Abstract】 Objective To evaluate the therapeutic effect of super selective chemoembolization of bronchial artery for the treatment of advanced lung cancer Materials and Methods Fifty patients with pathologically and radiologically confirmed advanced lung cancer were enrolled in this study. Arteriography of thoracic aorta was performed for mapping the vasculature,then selective catheterization of bronchial artery was performed to confirm the feeding artery of the tumor. Infusion of different anti-cancer drugs was carried out according to the pathological findings of the tumor, followed with embolization by lipiodol and gelatin sponge. Follow-up CT was performed at different duration after treatment. Results CR,PR,SD and PD were achieved in 4 (8%),26(52%),14(26%) and 6(12%) cases,respectively,resulting in CR+PR in 30 cases (60%) and CR+PR+SD in 44 cases (88%). 6 cases with progressive disease were noted. The clinical symptoms such as cough,chest pain,dyspnea and hemoptysis of 44 cases improved or disappeared after treatment. Hydrothorax (6 cases) and atelectasis (10 cases) were markedly improved as well. Obstructive pneumonia of 12 cases was recovered completely or partly .Conclusion Bronchial arterial chemoembolization is effective and reliable for the treatment of advanced lung cancer.更多还原
