肾上腺多层螺旋CT灌注成像的初步研究

【摘要】 目的研究多层螺旋CT灌注成像在肾上腺的临床应用。方法对5例正常肾上腺和10例经临床和病理证实的肾上腺病变(肾上腺增生4例,无功能肾上腺腺瘤3例,肾上腺转移瘤3例)进行多层CT灌注成像研究,利用CT灌注软件计算灌注参数,包括相对血流量(relative blood flow,RBF)、平均通过时间(mean transmit time,MTT)和表面通透性(permeability surface,PS)。结果正常肾上腺的RBF、MTT和PS分别为0.32ml/100g/min、50.33s、0.92ml/100g/min,肾上腺增生RBF、MTT和PS分别为0.23ml/100g/min、72.19s、0.89ml/100g/min,无功能腺瘤RBF、MTT和PS分别为0.13ml/100g/min、63.42s、0.85ml/100g/min,转移瘤RBF、MTT和PS分别为0.19ml/100g/min、45.31s、0.74ml/100g/min。结论肾上腺多层CT灌注成像是可行的肾上腺功能性检查,它反映了肾上腺微循环的血流灌注状况,为肾上腺的疾病的鉴别诊断提供有用的信息。

【Abstract】 Objective To study the clinical application of multi-slice CT perfusion imaging in adrenal gland. Methods Multi-slice CT perfusion imaging was perform in 5 healthy volunteers and 10 patients including adrenal hyperplasia (n=4),non-functioning adrenal adenoma (n=3) and adrenal metastasis (n=3). CT perfusion parameters including relative blood flow (RBF),mean transmit time (MTT) and permeability surface (PS) were analyzed. Results The RBF,MTT and PS of normal adrenal gland were 0.32 ml/ 100g/min,50.33s,0.92 ml/100g/min respectively. The RBF,MTT and PS of adrenal hyperplasia were 0.23 ml/100g/min,72.19s,0.89 ml/100g/min respectively. The RBF,MTT and PS of non-functioning adrenal adenoma were 0.13 ml/100g/min,63.42s,0.85 ml/100g/min respectively. The RBF,MTT and PS of non-functioning adrenal metastasis were 0.19 ml/100g/min,45.31s,0.74 ml/100g/min respectively. Conclusion Multi-slice CT perfusion imaging is a feasible functional examination for adrenal gland. Multi-slice CT perfusion imaging can reflect the microcirculatory blood flow of adrenal gland and provide useful information for differential diagonsis in adrenal diseases.
