502例神经导航系统辅助下幕上颅内肿瘤的显微手术

【摘要】 目的探讨神经导航系统辅助下颅内各部位肿瘤的显微手术切除方法,同时分析导航系统在颅内肿瘤切除中的作用。方法回顾性分析幕上颅内肿瘤采用显微导航手术治疗的502例患者资料。所有病例术前均行MRI T2 4 mm薄层全颅扫描,并将图片信息传送至Brainlab导航系统工作战,在工作站经转化、规划步骤将肿瘤标记,术中经注册后在导航系统指引下设计切口并在其辅助下切除肿瘤。结果肿瘤全切除408例,次全切除56例,部分切除32例,活检6例。结论神经导航系统能帮助医生精确规划手术计划,有效减少手术创伤,并能实时指导医生最大限度切除肿瘤,尽量减少对瘤周正常脑组织的损伤,提高肿瘤的全切率。

【Abstract】 Objective To explore the strategies of microsurgery assisted by neuronavigation for brain tumors,and to evaluate the effects of neuronavigation on resection of brain tumors.Methods 502 cases with brain tumors were treated by mrcrosurgery assisted by neuronavigation.All brain tumors were located in supra-tentorium,which histopathological categories included glioma,metastatic tumor,pituitary tumor and craniopharyngioma.All patients underwent MRI with T2 of slice 4mm,whose image data were worked out at BrainLab platform.The data were transmitted to neuronavigator in operation room,which indicated the plan of the cut line and resection of tumor.Results Total resection was achieved in 412 cases,subtotal resection in 56 cases,partial resection in 32 cases,biopsy in 6 cases.Conclusion Neuronavigation can help operator make precise surgical location of tumor,efficiently minimize surgical injury and improve the resection of tumor.
