p53及PCNA在子宫颈癌中的表达及临床意义

【摘要】 目的研究抑癌基因p53和增殖细胞核抗原(proliferating cell nuclear antigen,PCNA)在子宫颈癌中的表达特点,并探讨其与病理分级的关系。方法应用免疫组织化学ElivisionTM二步法检测35例宫颈癌及30例正常宫颈组织中p53及PCNA的表达情况,并结合病理分级进行分析。结果①P53蛋白在宫颈癌组织中的表达率为85.71%(30/35),明显高于在正常宫颈组织中的表达率6.67%(2/30),P=0.001;②P53蛋白阳性表达率与宫颈癌组织的病理学分级有关,P53在宫颈癌Ⅰ级中的阳性表达率57.14%(8/14)明显低于Ⅱ级86.67%(13/15)和Ⅲ级100.0%(6/6),P<0.05;③宫颈癌组织PCNA阳性表达率Ⅰ级64.29%明显低于Ⅱ级93.33%和Ⅲ级100.0%,P<0.05。结论p53基因突变在宫颈鳞癌发生中起重要作用,p53及PCNA检测对于研究宫颈癌的发生、发展和评估子宫颈癌恶性程度具有重要意义。

【Abstract】 Objective To study the expression of p53 and proliferating cell nuclear antigen(PCNA) proteins in primary cervical carcinomas and its clinic significance.Methods The expression of p53 and PCNA proteins in 35 cases with cervical carcinomas and 30 cases of normal cervical tissues was detected by supersensitive ElivisionTM immunohistochemical method.Results ①p53 positive expression rate in cervical carcinomas 85.71%(30 /35) was significantly higher than that in normal cervixes 6.67%(2 /30),P= 0.001.②p53 expression is correlated with the differentiation state and pathologic grades.The positive expression rate of p53 in the cervical carcinomas of grades Ⅰ 57.14%(8 /14) was significantly lower than that in the cervical carcinomas of grades Ⅱ 86.67%(13 /15) and grades Ⅲ 100.0%(6 /6),P<0.05.③The positive rate of PCNA in the cervical carcinoma of grade Ⅰ 64.29% was signifi cantly lower than that in the grade Ⅱ 93.33% and grade Ⅲ 100.0%,P<0.05.Conclusion Our data indicate that the alteration of p53 is involved in cervical carcinoma.Over expression of both p53 and PCNA may play important roles in the tumour genesis,progression and malignant behavior in cervical carcinomas.The detection of p53 proteins and PCNA can be used to evaluate malignant degree and prognosis of the cervical carcinoma.
