基于对应分析的冠心病心绞痛中医证候与合并病的相关性研究

【摘要】 目的:探讨冠心病心绞痛中医证候与合并病的相关性。方法:采用临床流行病学方法,多中心收集了500例经冠脉造影证实的冠心病心绞痛患者,用多元对应分析进行相关性研究。结果:在冠心病心绞痛的合并病中,高血压病、糖尿病与气阴两虚证的关系最为密切(相关距离0.08426、0.05810),血脂异常、脑卒中与气滞血瘀证关系最为密切(相关距离0.15068、0.31018)。结论:冠心病心绞痛的治疗除了针对其主要病机运用益气活血化痰方药之外,还应根据合并病的情况灵活辨证,适当配合养阴、疏肝解郁等方法,可望在中医药治疗冠心病心绞痛上获得更好的疗效。同时提示多元对应分析技术可能是研究中医证候复杂体系的较适合方法。

【Abstract】 Objective:To discuss the correlation of TCM syndrome and combined disease. Methods: 500 patients with angina pectoris in coronory heart disease confi rmed by coronary angiography were collected by clinical epidemiology. And research on the correlation of TCM syndrome and combined disease was carried out by correspondence analysis. Results:Among the combined diseases of angina pectoris in coronory heart disease,Hypertension is closely related to syndrome of deficiency of qi and yin,Miabetes is closely related to syndrome of deficiency of qi and yin too. Bloodlipid disturbance is closely related to qi stagnation and blood stasis,cerebral apoplexy is closely related to qi stagnation and blood stasis too. Conclusion:Besides the method of nourishing qi and quickening the blood and eliminating the phlegm, the method of nourishing yin and relieving the depressed Liver is also used in treating angina pectoris in coronory heart disease according to condition of combined disease. And treatment of angina pectoris in coronary heart disease with TCM may acquire better clinic effect. It is also suggested that multivariate correspondence analysis is a good method to research the complex system of TCM syndrome.
