【摘要】 目的探讨重建钢板内固定治疗跟骨关节内骨折的效果及减少并发症的策略。方法对35例39侧跟骨关节内骨折采用外侧扩大"L"形切口,使用跟骨重建钢板固定,并对术后并发症进行分析。结果术后3个月,35例39侧跟骨关节内骨折按Maryland评分,优良率为76.92%。Bhler角由术前13.2°±4.1°恢复到术后38.5°±9.1°。随访中有1例后关节面发生塌陷,Bhler角5°,行走严重疼痛。切口坏死2例,切口感染1例,无腓肠神经损伤。并发症发生率11.43%。结论采用重建钢板内固定治疗对SandersⅡ、Ⅲ型跟骨关节内骨折,术中掌握复位和固定的关键点,正确选择手术时机和切口无创操作能取得满意的疗效。更多还原

【Abstract】 Objective To study the reestablishment of plate internal fixation in the treatment of intraarticular calcaneal fracture and the way to reduce complications.Methods Thirty-five patients with intraarticular calcaneal fracture were treated with reconstruction plate internal fixation through an lateral-expansion "L" incision.The postoperative complications were analyzed.Results An assessment was done three months after operation based on Maryland score,excellent and good accounted for 76.92%.Bhler angle recovered from 13.2°±4.1° to 38.5°±9.1°.Posterior articular facet collapsed in one case with Bhler of five degrees and severe pain on walking.Two cases with incision necrosis,one with wound infection;no sural nerve was injured.The complication was 11.43%.Conclusion Plate internal fixation for Sanders Ⅱ and Ⅲ intraarticular calcaneal fracture will be satisfied as long as correct reduction and fixation,correct timing and correct manipulation are done.更多还原
