【摘要】 目的探讨节段性腰椎不稳症的手术改进方法和疗效。方法采用自体植骨(椎间扩孔结合自体椎板骨粒、棘突骨块植骨)椎间融合加椎弓根内固定治疗节段性下腰椎不稳或潜在不稳34例。结果34例均获得随访,随访时间8～38个月,平均16个月。按照日本JOA法疗效评定标准,优良率91%。X线片显示椎体间植骨融合良好,无高度及复位丢失。结论椎弓根内固定可矫正滑移,恢复脊柱的生理弧度,达到即刻稳定,增加植骨的融合率,提高手术疗效。而植骨方法的不同对椎间骨性融合成功与否关系极为密切。采用自体椎板、棘突及髂骨植骨减少了并发症,提高了融合率,节省了医疗费用,是治疗腰椎不稳的较好方法。更多还原

【Abstract】 Objective To investigate more perfect surgical operation methods and curative effect of treating abdominal lumbar vertebra unsteadiness. Methods Thirty-four cases with abdominal lumbar vertebra unsteadiness or latent abdominal lumbar vertebra unsteadiness were treated by autoallergic bone graft,vertebra-desis and lateral neck of vertebra’s internal fixation. Resealts With 8 months to 38 months follow-up , according to the curative effect assessment standard of Japanese JOA, good rate amounted to 91% and I-B bone graft’s coalesce were fine. Conclusion Lateral neck of vertebra’s internal fixation can rectify frift, recovery spinalis physiologic radian, increase confluens rate of bone graft and elevate curative effect.It’s a good methed of treating abdominal vertebra unsteadiness to use autoallergic vertebral plate, proces- sus spinosus and flank bone graft,which can decrease complication, elevate confluens rate and spare medical cost.更多还原
