64层螺旋CT前置心电门控冠状动脉成像临床应用初探
The study on clinical application of prospective ECG-triggered axial scan in coronary angiography by 64-slice CT
【摘要】 目的比较前置心电门控(前门控)与后置心电门控(后门控)64层螺旋CT冠状动脉(冠脉)成像(64 MSCTCA)的图像质量及放射剂量,探讨前门控对冠心病的临床应用价值。方法40例随即区分为两组,每组20例,平均心率一组为(63.75±10.58)次/分、一组为(66±9.4)次/分,分别进行64 MSCTCA前、后门控两组技术扫描;采用t检验方法对两种扫描方式的冠脉图像质量及有效放射剂量进行统计分析。结果前与后门控两组冠脉图像质量评分均值分别为(3.50±0.69)分和(3.35±0.75)分,两组间比较差异无统计学意义(P>0.05);前与后门控两组有效放射剂量均值分别为(2.40±0.02)mSv和(9.97±1.15)mSv,两组间比较差异有统计学意义(P<0.05),前门控低于后门控76.0%。结论在一定的心率(<70次/分)范围内,64层螺旋CT前门控冠脉成像的图像诊断质量与后门控具有一致性,且前门控明显降低了放射剂量。
【Abstract】 Objective To compare image quality and radiation dose between prospective electrocardiograph (ECG)-triggered axial scan and retrospective ECG-gated helical scan in coronary angiography by 64-slice CT(64 MSCTCA), and to investigate the clinical value of prospective electrocardiograph(ECG)-triggered axial scan using in the coronary heart disease.Methods Forty patients were devided into two groups randomly undergoing 64-slice CTA. 20 patients were taken prospective ECG-triggered axial scan and the orther were taken retrospective ECG-gated helical scan. The heart rate was (63.75±10.58) beats per minute(bpm) for group one and (66±9.14)bpm for group two, and both of them underwent 64 MSCTCA by prospective and retrospective ECG-gated scan.The image quality of coronary angiography and the effective radiation dose of two groups were analized by t-test.Results The mean image quality score of the prospective and retrospective ECG-gated groups were(3.50±0.69) and (3.35±0.75), and there was no significant statistical difference between them(P>0.05). The mean effective radiation dose of prospective retrospective ECG-gated groups were (2.40±0.02)mSv and (9.97±1.15)mSv, and showed significant statistical difference between the two groups (P<0.05).The mean effective radiation dose of prospective group was decreased by 76.0% than the retrospective one. Conclusion The image quality of diagnosis of prospective ECG-triggered axial scan in coronary angiography by 64-slice CT consistents with the retrospective one within a certain heart rate(<70bpm) range, and significantly reduced the radiation dose.
