18F-FDG SPECT显像区别阿尔茨海默病和血管性痴呆的临床研究
The value of 18F-FDG SPECT in distinguishing Alzheimer’s disease from vascular dementia
【摘要】 目的探讨阿尔茨海默病(AD)与血管性痴呆(VD)18F-脱氧葡萄糖(18F-FDG)单光子发射计算机断层(SPECT)的影像特点。方法静脉注射18F-FDG45min后行脑断层显像,获得脑显像特征,并加以分析、比较。结果22例AD患者可见双侧颞、顶、额叶对称或单侧优势半球的颞、顶、额叶异常代谢灶。20例VD患者除皮质各叶显示异常代谢灶外,合并基底节、丘脑、枕叶异常代谢灶,与基础病变(头颅CT或MRI定位)一致。结论18F-FDG SPECT可作为鉴别诊断AD、VD的有效手段。
【Abstract】 Objective To investigate the characteristics of 18F-FDG brain single-photon-emission computed tomography(SPECT) in both Alzheimer’s disease and vascular dementia.Methods Brain scan was performed 45min after intravenous administration of 18F-FDG SPECT and image data were analyzed and compared in both groups.Results In 22 Alzheimer’s disease patients there was symmetric hypometabolism in bilateral temple,parietal,frontal lobes or in temple,parietal,frontal lobes of dominant cerebral hemisphere in the 20 vascular dementia patients,heoreover there were hypometabolism foci in the basal ganglia,thalamus and occipital lobe,which were consistent with primary focus(by CT of MRI).Conclusions 18F-FDG SPECT images is an effective way to distinguish AD from VD.
