手背微型皮神经营养逆行岛状皮瓣的解剖观察及临床应用研究

【摘要】 目的探讨手背微型皮神经营养逆行岛状皮瓣的解剖特点及临床应用效果。方法解剖观测10具成年人体标本(20侧上肢)的手背皮神经及其伴行血管,设计出以手背皮神经伴行血管为蒂的逆行岛状皮瓣。临床应用35例36块皮瓣修复手指皮肤缺损。结果手背皮神经来源于桡神经浅支和尺神经手背支,在手背共有7条左右分支,均有相应的伴行血管,可设计7种逆行岛状皮瓣。本组运用了其中6种36块皮瓣,皮瓣大小为1.5cm×2cm-4cm×5cm。术后皮瓣全部成活,10例获随访1个月至1年,皮瓣色泽、质地、外形良好。结论手背微型皮神经营养逆行岛状皮瓣能够修复拇指以及其余四指近、中节皮肤缺损,皮瓣血供可靠,创伤小,操作简单,是一理想供区。

【Abstract】 Objective To study an anatomical basis for dorsal neurocutaneous reverse island mini-flap and its clinical value. Methods dorsal cutaneous nerve and its comites were studied in 10 adult cadavers(20 hands). The dorsal neurocutaneous reverse island mini-flaps was designed.35cases and 36flaps were used to heal cutaneous deficiency on fingers. Results dorsal neurocutaneous were origined from either superficial branch of radial nerve or dorsal cutaneous branch of ulnar nerve,about 7 branches and its comites on it.7 different kinds flaps were designed.36 flaps of 6 different kinds flaps were applied in the group. The area of flaps was 1.5cm×2cm~4cm×5cm . All the flaps were survived. 10 patients were followed up one month to one year.The color,texture , appearance and function of the flaps were good. Conclusions The dorsal neurocutaneous reverse island mini-flap can repair the cutaneous deficiency of thumb and proximalphalanx and midplate of other four fingers . Blood supply of flaps was reliable, injury to hand was minor, the operation was easy.It’s a ideal donor.
