【摘要】 目的探讨CT灌注成像与MELD评分对肝移植前肝功能评价的相关性及意义。方法选择22例CTP分级C级行肝移植的肝硬化患者及20例对照者,行多层螺旋CT动态增强扫描,计算肝移植前HAP、HPP、TLP和HAI,评价肝硬化患者的各项CT灌注参数与MELD评分之间的相关性。结果肝移植前肝硬化组HAP、HPP、TLP、HAI(%)分别为(38.85±4.09)、(59.77±7.97)、(98.63±8.83)ml/(min·100ml)和(39.55±4.31)%。慢性终末期肝病患者肝脏的灌注值与临床评价肝功能的MELD评分存在负相关的关系,MELD与HPP(r=-0.814,P=0.000)、MELD与TLP(r=-0.775,P=0.000)。结论CT灌注参数可反应肝移植前肝脏的血流灌注情况,其与MELD评分存在负相关关系。更多还原

【Abstract】 Objective To discuss the correlation and significance of CTPI and the MELD score for liver function evaluation before liver transplantation.Methods Twenty-two cases of liver cirrhosis with CTP C stage before liver transplantation and 20 control subjects underwent MSCT,and the volume of HAP,HPP,TLP and HAI were accounted to evaluate the correlation of CT perfusion parameters and MELD score for patients with cirrhosis.Results HAP,HPP,TLP and HAI were(38.85±4.09),(59.77±7.97),(98.63±8.83)ml/(min·100 ml)and(39.55±4.31)% respectively for patients with liver cirrhosis CTP C stage.Liver perfusion and clinical evaluation of liver function MELD score were negatively correlated for chronic liver disease patients in the end-stage,MELD and HPP(r=-0.814,P=0.000),MELD and TLP(r=-0.775,P=0.000).Conclusion CT perfusion parameters can reflect abnormal perfusion before liver transplantation.There is a negative correlation between liver perfusion parameters and the MELD score.更多还原
