【摘要】 目的探讨螺旋CT增强扫描技术在引导近肺门区肿块病灶穿刺活检中的应用价值。方法比较45例CT增强(Ⅰ组)和69例CT平扫(Ⅱ组)引导下近肺门区肿块病灶穿刺活检的成功率、肺癌分型、分级准确率及并发症发生率。结果Ⅰ组、Ⅱ组穿刺成功率分别为91.1%、79.7%,二者差异无统计学意义(P>0.05)。两组肺癌分型、分级准确率分别为91.2%、71.7%,Ⅰ组明显高于Ⅱ组(P<0.05)。两组穿刺后并发症发生率分别为17.8%、36.2%,Ⅰ组明显低于Ⅱ组(P<0.05)。结论螺旋CT增强扫描技术引导近肺门区肿块病灶穿刺活检,可降低穿刺风险性、提高肺癌分型、分级准确率及穿刺成功率。更多还原

【Abstract】 Objective To explore the value of postcontrast spiral CT scan technique in guiding biopsy of lumps near pulmo- nary hilum.Methods A total of 114 patients with lumps near pulmonary hilum underwent CT guided biopsy.These biopies were guided either by postcontrast CT in 45 patients(groupⅠ),or by non-contrast CT in 69 patients(groupⅡ),and the success rate of biopsy,the accuracy in the classification and grading of lung cancer and the incidence of complication were compared.Results The success rate of biopsy for groupⅠandⅡwas 91.1%(41/45)and 79.7%(55/69),respectively. The accuracy in classification and grading of lung cancer for groupⅠandⅡwas 91.2%(31/34)and 71.70/oo(33/46),re- spectively.The incidence of complication for groupⅠwas 17.8%(8/45),and for groupⅡwas 36.2 %(25/69).Conclusion Postcontrast spiral CT guided biopsy of lumps near pulmonary hilum can increase the accuracy in classification and grading of lung cancer with lower risk and higher success rate.更多还原
