MSCT平扫诊断双上腔静脉

【摘要】 目的分析双上腔静脉的MSCT表现,评价MSCT平扫在双上腔静脉(DSVC)诊断中的价值。方法回顾性分析12例无上腔静脉回流相关症状病例,行16层螺旋CT平扫,在ADW4.2工作站进行重组,观察其影像特点和走行,并测量双侧上腔静脉的长径、短径和横断面面积。结果①左上腔静脉(LSVC)表现为主动脉弓左侧椭圆形软组织密度结节影,上下至少3个连续层面都可以见到,可合并冠状窦扩张。②重组影像表现为纵隔左侧与正常右侧上腔静脉平行的管状影,依次走行于主动脉弓左侧、左上肺静脉前方、左心房左后方,经增粗的冠状窦汇入右心房。结论MSCT平扫可明确诊断双上腔静脉,心脏上方层面能观察LSVC连接、走行及毗邻。心脏层面部分病例可显示汇入部位。

【Abstract】 Objective To analyze the MSCT appearance of double superior vena cava (DSVC), and to evaluate the role of MSCT plain scan in the diagnosis of DSVC. Methods Twelve cases of DSVC without symptoms related to its drainage were analyzed retrospectively. All cases received MSCT plain scan. Thin slice images were reformatted on ADW 4.2 workstation. The imaging characteristics and pathway were observed. The long diameter, short diameter and cross section area were also measured. Results ①Left superior vena cava manifest as nodules in the left side of aortic arch, and can be seen on at least three continuous slices. Coronary sinus expansion can also be seen. ②On reformatted images, the LSVC manifest a canal along the left side of the midiastinum that was parallel to the normal right side SVC. The LSVC located in the left side of the aortic arch, behind the left superior lung vein, left and behind of the left artrium, respectively. Then drainaged into right atrium through the enlarged coronary sinus. Conclusion MSCT plain scan can not only give the proper diagnosis of DSVC, but also display the connection, pathway and adjacent structure. At the heart level, only part of the cases can display the site of abouchement.
