【摘要】 目的通过对15例SARS后被确诊为Ⅰ期骨缺血坏死后行高压氧治疗病例的追踪研究,探讨高压氧治疗SARS后Ⅰ期股骨头缺血坏死的疗效。方法符合WHO确定的SARS诊断标准的15例患者(23髋)行双侧股骨头影像学检查,其中平片、CT检查为阴性,MRI诊断为Ⅰ期股骨头缺血性坏死。高压氧治疗后,随诊34个月,对比治疗前后平片、CT和MRI结果。结果在高压氧治疗前,所有患者股骨头缺血性坏死情况根据1992年国际骨循环联合会(ARCO)国际分期标准均诊断为Ⅰ期,高压氧治疗后随访近3年,无痊愈者,2例(3髋)进展为Ⅳ期,1例(1髋)进展为Ⅲ期,12患者(19髋)进展为Ⅱ期。结论高压氧治疗不能逆转或有效阻止SARS后Ⅰ期股骨头缺血性坏死的进展。更多还原

【Abstract】 Objective To evaluate the treatment outcome of hyperbaric-oxygen-therapy on stage Ⅰ avascular necrosis of the femoral heads (AVN) in SARS patients.Methods Fifteen patients(23 hips) who fitted the WHO case definition for SARS were identified to have stage Ⅰ AVN by MRI.No abnormality could be found in conventional radiographs and CT of the hips.All patients underwent the HBO therapy.After 34 months we compared the results of X-ray,CT and MRI.Results Before the treatment of HBO,all of the patients with AVN of femoral heads were at stage Ⅰ according to the system of Steinberg.After about 3 years follow-up,none of the hips with AVN demonstrated evidence of complete resolution.Two patients(3 hips) progressed to stage Ⅳ AVN,one patient(1 hip) progressed to stage Ⅲ AVN;other patients(12 patients,19 hips) progressed to stage Ⅱ AVN;Conclusion HBO can not heal or prevent the progress of avascular necrosis of the femoral head in SARS patients.更多还原
