盐酸戊乙奎醚用于小儿氯胺酮-丙泊酚静脉麻醉前用药的临床研究
Randomized controlled clinical study on Penehyclidine hydrochloride as a premedication in the ketamine TIVA in children
【摘要】 目的评价盐酸戊乙奎醚作为术前用药在小儿静脉全麻中对心血管和腺体分泌等的影响。方法40例3~10岁患儿随机分为两组,麻醉前30min分别肌肉注射盐酸戊乙奎醚(P组)或阿托品(A组),观察并记录各时点的心率(HR)、平均动脉压(MAP)、呼吸(R)、唾液分泌量(SS)及不良反应等指标。结果P组和A组SS在给药后20、30和60min时均显著低于0min时,而P组在150min时亦显著低于0min时及A组;P组在给药后各时间点的MAP、HR及R和0min时差异无显著性,A组HR在给药后20、30和60min时升高,MAP在给药后30和60min时亦显著升高,两者在同时点均高于P组。结论盐酸戊乙奎醚能较长时间抑制呼吸道腺体分泌,且对心率及血压几无影响,比阿托品更适合用于小儿术前用药。
【Abstract】 【Objective】To evaluate the effects of Penehyclidine hydrochloride on respiration and glandular secre-tions when premedicated in children.【Methods】 40 patients aged 3-10 years were randomly divided into two groups.Penehyclidine hydrochloride(group P) or Atropine(group A) was premedicated intramuscularly 30 min before anesthesia.Heart rate(HR),mean arterial pressure (MAP),breath rate(R) and the amount of saliva secretion(SS) were recorded before and after premedication.【Results】 SS reduced significantly 20 min,30 min and 60 min after pre-medication in both groups,and in 150 min,it was still in a significantly reduce level in group P,which was signifi-cantly lower than that in group A.MAP,HR and R in group P showed no significant differences before and after premedication.But in group A,HR increased significantly at 20 min,30 min and 60 min after premedication,MAP increased significantly at 30 min and 60 min after premedication,meanwhile,both of them were also significantly higher than those in group P.【Conclusions】Penehyclidine hydrochloride can effectively reduce respiratory glandular secretion,and nearly has no influence on heart rate and blood pressure,which suggests it could be a superior at-ropine alternative as an anesthesia premedication in children.
