中药清热消炎固齿缓释剂对牙周炎患者龈下菌群及龈沟液内毒素水平的影响
Effect of traditional Chinese medicine Qing Re Xiao Yan Gu Chi local slow-release delievery agent on subgingival bacteria and levels of endotoxin in gingival crevicular fluid of periodontitis
【摘要】 目的观察中药清热消炎固齿缓释剂对成人牙周炎患牙龈下菌群及龈沟液中内毒素水平的影响。方法选择86例牙周炎患者,随机分为实验组和对照组,每组各43例86个患牙。基础治疗后,实验组牙周袋内置放清热消炎固齿缓释剂,每周1次,共3次,对照组不放任何药物,两组分别于治疗前和第3周末检测其龈沟液中内毒素的含量及龈下菌群比例变化。结果两组治疗后龈沟液中内毒素水平较治疗前明显降低,实验组与对照组相比差异有显著性(P<0.01)。两组治疗后龈下菌斑中球菌比例较治疗前均明显增加;杆菌、螺旋体比例较治疗前均明显降低(P<0.01或P<0.05);实验组与对照组相比差异有显著性(P<0.01)。结论基础治疗合并清热消炎固齿缓释剂局部应用对患牙龈下菌群及龈沟液中内毒素水平有明显影响。
【Abstract】 【Objective】 To evaluate the effect of the traditional Chinese medicine Qing Re Xiao Yan Gu Chi lo-cal slow-release delievery agent on the subgingival bacteria and levels of endotoxin in gingival crevicular fluid of adult periodontitis. 【Method】 86 adult periodontitis with periodontitis(172 teeth) were divided into two groups (43 patients with 86 teeth each) randomly. The experimental group was treated with Qing Re Xiao Yan Gu Chi local slow-release delievery agent in the periodontal pocket once a week for three weeks while the control group with nothing after basic treatment. Levels of endotoxin in gingival crevicular fluid and the proportion of subgingival bac-teria were measured before and after 3 weeks′ therapy. 【Results】 After therapy, levels of endotoxin in gingival crevicular fluid decreased remarkably in two groups. There were significant differences between the two groups(P < 0.01).The proportion of subgingival coccoids greatly increased ,whereas that of bacillus and spirochetes decreased significantly in two groups (P <0.01 or P <0.05). There were significant differences between the two groups (P < 0.01). 【Conclusions】 Basic treatment combining local application of the traditional Chinese medicine Qing Re Xi-ao Yan Gu Chi local slow-release delievery agent had apparent effect on the subgingival bacteria and levels of endo-toxin in gingival crevicular fluid of adult periodontitis.
