高频超声诊断腋窝淋巴结增大临床价值

【摘要】 目的探讨腋窝淋巴结增大的二维高频超声彩色多普勒超声表现特点,对良恶性病变的鉴别意义。方法应用高频超声检查48例乳腺癌患者269个腋窝淋巴结,对其二维(2D)高频超声及彩色多普勒(CDFI)超声所见与病理结果进行对照分析。结果超声检查发现并获得病理诊断的淋巴结269个,其中良性增大淋巴结103个,转移性癌淋巴结82个,正常淋巴结84个。2D表现:良性反应性增生(36个),表现为皮质均匀增厚,淋巴门血供呈树枝状增多。良性脂肪浸润(67个),表现为淋巴结回声增强,边界欠清,血供不丰富。转移性癌淋巴结结构不清,血供丰富而紊乱。结论依据2D及CDFI表现特点,有助于腋窝淋巴结良恶性病变的鉴别诊断。

【Abstract】 [Objective] To investigate the Doppler high frequency two-dimensional ultrasonographic characteristics of different axillary lymphatic nodes and differential value for benign and malignance. [Methods] Two hundred and sixty-nine axillary lymphatic nodes in 48 patients with breast carcinoma were examined using Doppler high frequency ultrasound, the ultrosonographic and CDFI features of these lesions were studied compared with pathologies. [Results] Of the 269 axillary lymphatic nodes confirmed by pathology, 103 lymphatic nodes were benign enlargement, 82 metastatic lesions, 84 normal. Features of color Doppler sonographies of the lymphtic nodes were: cortex thickened evenly in reactive proliferative lymphatic nodes (36 lymphatic nodes), with increased dendritic pattern blood signal in lymphatic hilum; enhanced echo and obscure circumscribe in nodes of benign fat infiltration (67 lymphatic nodes), with little blood signal in lymphatic hilum. Structure of lymphatic nodes of metastatic carcinomas was blurring, with abundant turbulent blood signal. [Conclusion] Two-dimensional ultrasound and CDFI can provide useful information for the differentiation of benign and malignance of lymphatic nodes.
