【摘要】 目的探讨腹腔镜下卵巢巧克力囊肿剔除术中创面采用缝合法止血对术后卵巢储备功能的影响。方法2001年6月~2006年8月,对36例卵巢巧克力囊肿在腹腔镜下剔除囊肿后,术中创面采用缝合法止血。术前、术后第2次月经第2~3天及术后6个月第1次月经第2~3天分别抽血检测卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)及阴道B超探测窦状卵泡数(Fo),以判断卵巢储备功能。结果①FSH、LH、E2在术前、术后及术后6个月比较差异均无显著性(P>0.05)。②术后第2次月经和术后6个月第1次月经Fo分别为(5.0±1.5)个及(5.2±1.5)个,较术前(6.1±1.2)个明显减少(q=4.690、3.838,P<0.05)。③术后2个时点基础FSH≥12IU/L的例数(5例和3例)均多于术前(0例),但无统计学意义(χ2=3.439,P=0.064;χ2=1.391,P=0.238)。未发现FSH≥40IU/L的患者。结论腹腔镜下卵巢巧克力囊肿剔除术中缝合止血法未见影响卵巢储备功能。更多还原

【Abstract】 Objective To investigate the influence of suture hemostasis on ovarian reserve after laparoscopic cystectomy for ovarian chocolate cyst. Methods From June 2001 to August 2006, 36 patients with endometriotic ovarian cyst were treated with laparoscopic cystectomy, suture hemostasis was performed during the operation. Blood samples were obtained from the patients before the operation (group 1), during the second menstrual cycle postoperation (on day 2 or 3,group 2), and during the first menstrual cycle 6 months after the operation (on day 2 or 3, group 3). The serum levels of FSH, LH, E2 were tested, and basal antral follicle number (Fo) was detected by transvaginal B ultrasonography to determine the ovarian reserve. Results ① No significant differences were detected in the serum levels of FSH, LH, and E2 among the three groups (P>0.05). ② The Fo in groups 2 and 3 were (5.0±1.5) and (5.2±1.5) respectively, which were significantly lower than that in group 1 (6.1±1.2)(q=4.690, 3.383, P<0.05). ③ The patients with a FSH level ≥12 IU/L in groups 2 (5 cases) and 3 (3) were more than that in group 3 (0), however, the difference was not significant (χ2=3.439, P=0.064; χ2=1.391, P=0.238). Conclusion Suture hemostasis does not influence the ovarian reserve after laparoscopic cystectomy for ovarian chocolate cyst.更多还原
