低级别脑胶质瘤的手术时机:18例动态影像学观察

【摘要】 目的总结低级别胶质瘤的生长、扩散规律,为手术时机的选择提供参考。方法分析18例低级别胶质瘤的术前影像学表现。根据病灶部位变化与否,将病例分为两组:扩散组和未扩散组,比较两者的检查周期是否有明显差别。结果星形细胞瘤6例,少枝星形细胞瘤2例,少枝胶质细胞瘤10例。平均检查周期为53.1个月(5 ̄118个月)。9例病变在原始部位缓慢生长,9例自原发部位向其他部位扩散。扩散组的检查周期大于未扩散组(t=-3.622,P<0.05)。结论低级别胶质瘤倾向于自初始部位向其他部位扩散。低级别胶质瘤可择期手术,在此期间定期随访是非常必要的。

【Abstract】 Objective To explore the regularities of growth and extention in cerebral low-grade glioma for providing reference for the choice of surgical timing. Methods Preoperative imaging appearances of 18 low-grade gliomas were analyzed. According to the lesion site, all of the cases were divided into two groups with their examination period compared. Results There were 6 astrocytomas, 2 oligoastrocytomas, and 10 oligodendrogliomas in these cases. The mean examination period was 53.1 months (range 5-118 months). Nine of 18 patients showed slow growth the tumor on the primary site, and the other 9 had dissemination of tumor cells. In the group with dissemination, the mean examination period was longer than the group without dissemination (t = -3.622, P < 0.05). Conclusion Low-grade gliomas show a tendency of dissemination of tumor cell to other sites. Selective operation is practicable and regular follow-up is recommended before operation.
